重度慢性阻塞性肺疾病患者贫血与肺功能的关系

摘　　要：目的研究重度慢性阻塞性肺疾病(简称"慢阻肺")患者中贫血与肺功能的关系。方法 2018年1月至2019年10月收集本院收治的189例重度慢阻肺患者。按照患者血红蛋白含量分为贫血组和非贫血组。比较两组患者血常规、血气分析、肺功能等结果。并运用logistic回归分析贫血的影响因素。结果在收集的189例重度慢阻肺患者中有61例贫血患者,贫血发生率为32.28%。贫血组的PFT参数FEV1、DLCO的结果低于非贫血组,而PFT其他参数贫血组与非贫血组差异均无统计学意义。贫血组的CPET参数Peak Load,Peak VO2、Peak O2 pulse结果均低于非贫血组(P<0.05)。logistic回归分析发现年龄是贫血发生的影响因素。在贫血组中峰值摄氧量与血红蛋白浓度呈正相关。结论慢阻肺患者的血红蛋白水平越高,其肺功能越好。Objective To investigate the relationship between anemia and pulmonary function in patients with chronic obstructive pulmonary disease(COPD).Methods 189 cases of patients with COPD admitted and diagnosed in our hospital from January 2018 to October 2019 were selected as the research objects,including 61 anemic patients and 128 non-anemic patients.Blood routine test,blood gas analysis and lung function were compared between the two groups.Logistic regression was used to analyze the influencing factors of anemia.Results Among the 189 patients with severe COPD,61 were anemic,the incidence of which was 32.28%.The results of FEV1 and DLCO in the anemia group were lower than those in the non anemia group,but the other parameters of the PFT were not significantly different between the anemia group and the non-anemia group.The results of peak load,peak VO2 and peak O2 pulse in the anemia group were lower than those in the non anemia group.Logistic regression analysis foundthat age was the influencing factor of anemia.The peak oxygen uptake was positively correlated with hemoglobin concentration.Conclusion The higher hemoglobin value in patients with COPD,the better the pulmonary function.
