终末期肾病腹膜透析患者继发不安腿综合征的影响因素分析

摘　　要：目的探讨终末期肾病维持性腹膜透析(continuous ambulatory peritoneal dialysis,CAPD)患者不安腿综合征(restless legs syndrome,RLS)的发生情况及其影响因素,为临床防治RLS提供依据。方法选取接受CAPD治疗的482例终末期肾病患者,采用Logistic回归分析RLS发生的相关影响因素。结果482例CAPD患者中,发生RLS的为108例(22.4%)。非RLS组与RLS组患者合并糖尿病比例、贫血用药情况比例,血肌酐、P、K、β2-微球蛋白、PTH水平比较差异均有统计学意义(P<0.05)。多因素Logistic回归分析结果显示,合并糖尿病(OR=4.008)、血肌酐(OR=1.126)、β2-微球蛋白(OR=1.735)、PTH(OR=1.047)是发生RLS的危险因素(P<0.05)。结论RLS在维持性腹膜透析的终末期肾病患者中具有较高的发病率,合并糖尿病、β2-微球蛋白、PTH是维持性腹膜透析的终末期肾病患者RLS发生的危险因素。Objective To investigate the incidence and influencing factors of restless legs syndrome(RLS)in patients with end-stage renal disease(ESRD)undergoing continuous ambulatory peritoneal dialysis(CAPD),so as to provide basis for clinical prevention and treatment of RLS.Methods A total of 482 patients with ESRD treated with CAPD were included.Logistic regression was used to analyze the related factors of RLS.Results Among 482 CAPD patients,108 cases(22.4%)developed RLS.There were significant differences in diabetes mellitus,anemia medication,serum creatinine,P,K,β2-microglobulin and PTH levels between non-RLS group and RLS group(P<0.05).Multivariate logistic regression analysis showed that diabetes mellitus(OR=4.008),serum creatinine(OR=1.126),β2-microglobulin(OR=1.735)and PTH(OR=1.047)were the risk factors for RLS(P<0.05).Conclusions RLS had a high incidence in ESRD patients undergoing CAPD.Diabetes,β2-microglobulin and PTH were the risk factors for RLS in ESRD patients undergoing CAPD.
