关节腔注射玻璃酸钠与富血小板血浆治疗膝关节骨性关节炎的疗效比较

摘　　要：目的对比分析关节腔内注射玻璃酸钠与自体富血小板血浆(PRP)治疗膝关节骨性关节炎的愈后和关节功能评分。方法回顾性分析2016年12月—2019年11月间贵港市人民医院骨科门诊治疗膝关节骨性关节炎患者97例,其中关节腔注射玻璃酸钠治疗52例(SH组)和关节腔注射自体富血小板血浆治疗45例(PRP组)。两组治疗后规范康复和定期随访,比较两组患者治疗前、后膝关节疼痛和关节功能评分变化,并以此评价两种治疗方法的临床效果。结果治疗期间两组患者无死亡病例,均获得6个月以上随访,平均7.18个月。治疗前两组患者的VAS评分及WOMAC评分比较差异无统计学意义(P>0.05);与治疗前比较,两组治疗后1个月、3个月、6个月的VAS评分及WOMAC评分显著下降(P<0.05),PRP组治疗后3个月、6个月的VAS评分及WOMAC评分显著低于SH组(P<0.05)。结论关节腔内注射自体富血小板血浆治疗膝骨性关节可有效减轻关节疼痛症状并改善膝关节功能,并可获得较长期良好地愈后,提高患者满意度,是治疗膝关节骨性关节炎保守治疗的较理想方法。Objective To compare and analyze the prognosis and joint function scores of intra-articular injection of sodium hyaluronate and platelet rich plasma(PRP)in the treatment of knee osteoarthritis.Methods Retrospective analysis was conducted on 97 patients with knee osteoarthritis treated in the Department of Orthopedics,Guigang People’s Hospital from December 2016 to November 2019,among which 52 were treated with intra-articular injection of sodium hyaluronate(SH group)and 45 were treated with intra-articular injection of autologous platelet rich plasma(PRP group).After treatment,patients in two groups received standardized rehabilitation and regular follow-up,and the changes in knee pain and joint function scores before and after treatment were compared between two groups,so as to evaluate the clinical effects of two treating methods.Results During treatment,no death occurred in both groups.All patients were followed up for more than 6 months(average 7.18 months).There was no significant difference in VAS score and WOMAC score between two groups before treatment(P>0.05).VAS score and WOMAC score decreased significantly in two groups after 1 month,3 months and 6 months compared with those before treatment(P<0.05).VAS scores and WOMAC scores of PRP group at 3 and 6 months after treatment were significantly lower than those of SH group(P<0.05).Conclusion Intra-articular injection of platelet rich plasma can effectively relieve arthralgia symptom and improve the function of knee joints.As an ideal conservative treatment for knee osteoarthritis,it can obtain a longer period of good recovery and improve patient satisfaction.
