摘 要:目的探讨超声不同感兴趣区(region of interest,ROI)勾画大小对剪切波弹性成像诊断乳腺肿瘤的影响。方法63例乳腺肿块患者,共73个肿块,术前行常规超声及剪切波弹性成像(shear wave elastography,SWE)检查,获得肿块BI-RADS分类。SWE检查采用两种ROI(ROI-L和ROI-S),分别测弹性最大值(Emax)和弹性平均值(Emean),采用ROC曲线比较不同ROI选取方法对弹性参数诊断效能的影响,探讨BI-RADS分类联合SWE诊断乳腺肿块的价值。结果SWE检查不同ROI大小选择对Emean诊断乳腺肿瘤的效能有显著影响(P=0.030),对Emax诊断乳腺肿瘤的效能无影响,差异无统计学意义(P=0.077)。BI-RADS分类联合SWE诊断效能显著高于单独使用BI-RADS分类的诊断效能。结论不同ROI大小对Emean诊断效能有一定影响,对Emax诊断效能无显著影响。BI-RADS分类基础上联合SWE可提高乳腺肿瘤诊断效能,具有较高的应用价值。Objective To evaluate the influence of region of interest(ROI)size on the diagnostic performance of shear wave elstography(SWE)of breast tumors.Methods Total sixty-three patients with seventy-three breast masses were examined by conventional ultrasound and SWE before surgery,and the BI-RADS classification of breast masses were obtained.Two kinds of ROI(ROI-L and ROI-S)were used in SWE examination.The elastic maximum stiffness(Emax)and elastic mean stiffness(Emean)of these two kinds of ROI were measured respectively.ROC curves were used to compare the effects of different types of ROI on the diagnostic performance of SWE elastic parameters.Then,the value of BI-RADS classification combined with SWE in the diagnosis of breast masses was explored.Results Different ROI sizes in SWE examination had a significant effect on the diagnostic performance of Emean in breast tumors(P=0.03),but had no effect on the diagnostic performance of Emax in breast tumors,and the difference was not statistically significant(P=0.077).The diagnostic performance of BI-RADS classification combined with SWE was significantly higher than that of BI-RADS classification alone.Conclusion The ROI size affects the diagnostic performance of Emean,and donot affect the diagnostic performance of Emax.BI-RADS classification combined with SWE can improve the diagnostic performance of breast tumors and has high application value.
