雷波县中学教师膝关节结构改变的放射学调查

摘　　要：目的通过放射检查方法,探讨凉山彝族自治州雷波县中学教师群体膝关节结构改变的程度。方法招募符合标准的雷波县在职初、高中教师,在2019-10-14/24进行集中检查:包括膝关节立位及卧位正位、卧位侧X光摄片,并测量立位、卧位膝关节间隙,立位胫骨角与股胫角,侧位LT/LP值,并进行双侧膝关节同侧及双侧交叉检验。结果共招募96位教师。右膝关节立位内、外侧关节间隙值为(5.93±1.03)mm、(5.98±1.01)mm,右膝关节卧位内、外侧关节间隙值为(6.07±0.97)mm、(6.14±0.98)mm。左膝关节立位内、外侧关节间隙值为(6.05±0.92)mm、(6.07±0.93)mm,左膝关节卧位内、外侧关节间隙值为(6.11±0.93)mm、(6.12±0.9)mm。存在卧位关节间隙宽度大于立位,且无论立位或卧位,内侧关节间隙宽度均小于外侧的趋势,右膝磨损程度大于左膝。右膝胫骨角与股胫角值为(94.82±2.59)°、(174.30±2.83)°;左膝胫骨角与股胫角值为(94.23±2.13)°、(173.78±2.96)°,约一半教师存在膝内翻。LT/LP值为:右膝(1.09±0.09),左膝(1.10±0.13),13位教师存在可疑髌骨高位。结论长期站立及不正确的坐立行走姿势,引起雷波县教师群体膝关节较为明显的结构改变,应引起重视并积极保持正确姿态。Objective To explore the degree of structural change of knee joint of teachers by X-ray examination in Leibo County in Liangshan Yi Autonomous Prefecture.Methods In-service junior and senior high school teachers meeting the standards were recruited in Leibo County.Centralized inspection of participants was carried out from October 14 to 24 in 2019.X-ray films were taken from the standing and lying positions of the knee joint,and the knee joint space,tibial angle and femoral tibial angle,and LT/LP values were measured.Two-sided t tests were used to examine the ipsilateral and bilateral cross examination of bilateral knee joints.Results A total of 96 teachers were recruited.The clearance values of the medial and lateral joint space in the upright position of the right knee were(5.93±1.03)mm and(5.98±1.01)mm.Similarly,the clearance values of the medial and lateral joint space of the right knee joint in lying position were(6.07±0.97)mm,(6.14±0.98)mm.As for the left knee joint,the values of the internal and external joint space in the upright position were(6.05±0.92)mm and(6.07±0.93)mm,and the values of the internal and external joint space in the lying position were(6.11±0.93)mm and(6.12±0.9)mm.There was an trend that the width of the joint space in the supine position was larger than that in the vertical position,and the width of the joint space in the medial position was smaller than that in the lateral position.The wear degree of the right knee is larger than that of the left knee.The tibial angle and the femoral tibial angle of the right knee were(94.82±2.59)°and(174.30±2.83)°,respectively.The tibial angle and the femoral tibial angle of the left knee were(94.23±2.13)°and(173.78±2.96)°,respectively.About half of the teachers had genu varus.The LT/LP values for right knee and left knee were(1.09±0.09)and(1.10±0.13),respectively.Totally,13 teachers had suspicious patellar high position.Conclusion Long-term standing and incorrect sitting and walking posture resulted in obvious structural changes of
