髋关节置换术后超声神经刺激器引导下股神经阻滞麻醉对患者术后镇痛的效果探讨

摘　　要：目的探讨髋关节置换术后超声神经刺激器引导下股神经阻滞麻醉对患者术后镇痛的效果。方法 56例实施髋关节置换术患者,随机分为基础组和干预组,各28例。基础组术后镇痛泵镇痛,干预组术后镇痛泵联合超声神经刺激器引导下股神经阻滞麻醉镇痛。比较两组患者术后疼痛情况、镇痛泵按压次数、曲马多用量。结果干预组患者术后12、24、72 h疼痛评分分别为(2.6±0.5)、(3.2±0.6)、(2.8±0.4)分,均低于基础组的(3.5±0.9)、(4.2±1.1)、(3.7±0.7)分,差异有统计学意义(P<0.05)。干预组镇痛泵按压次数(3.1±1.1)次及曲马多用量(112.3±21.1)mg均少于基础组的(7.2±1.6)次、(252.4±32.6)mg,差异有统计学意义(P<0.05)。结论髋关节置换术后超声神经刺激器引导下股神经阻滞麻醉能减轻患者术后疼痛程度,减少镇痛药物使用量。Objective To discuss the postoperative analgesia effect of femoral nerve block anesthesia guided by ultrasonic nerve stimulator after hip replacement. Methods A total of 56 patients with hip replacement were randomly divided into basis group and intervention group, with 28 cases in each group. The basis group received analgesia pump for postoperative analgesia, and the intervention group received femoral nerve block anesthesia guided by ultrasound nerve stimulator. The postoperative pain situation, number of analgesic pump compressions and the dosage of tramadol were compared between the two groups. Results In postoperative 12, 24 and 72 h, the pain score of the intervention group were(2.6±0.5),(3.2±0.6) and(2.8±0.4) points, which were lower than(3.5±0.9),(4.2±1.1) and(3.7±0.7) points of the basis group, and the difference was statistically significant(P<0.05). The number of analgesic pump compressions(3.1±1.1) times and the dosage of tramadol(112.3±21.1) mg of the intervention group were less than(7.2±1.6) times and(252.4±32.6) mg of basis group, and the difference was statistically significant(P<0.05). Conclusion Femoral nerve block anesthesia guided by ultrasonic nerve stimulator after hip replacement can reduce the pain of patients as well as the dosage of analgesics.
