摘 要:目的应用基于体素的形态学测量(VBM)技术及局部一致性(ReHo)值分析2型糖尿病(T2DM)患者大脑结构及功能异常的相关性。方法对25例T2DM患者(T2DM组)及33例健康志愿者(HC组)进行常规MRI、脑部静息态MRI及3D全脑高分辨率解剖像扫描。通过处理静息态图像数据获得2组受试者的ReHo值并采用SPM软件对全脑高分辨率解剖图像进行处理获得大脑灰质体积,然后对受试者的ReHo值及大脑灰质体积进行相关性分析。结果与HC组相比,T2DM组左侧海马区、左侧梭状回、左侧颞上回、左侧尾状核、右侧额上回、右侧额中回、右侧楔前叶、右顶叶及右侧丘脑ReHo值均减低(P<0.05);左侧颞横回、左侧岛叶、左侧颞中回、右侧梭状回、右侧舌回、右侧枕叶及右侧扣带回VBM均减低(P<0.05)。结论T2DM患者大脑灰质体积变化的脑区与功能异常的脑区未出现重叠,提示T2DM患者大脑灰质体积的变化与功能的改变可能是相互独立的。Objective To investigate the correlation between brain structure and dysfunction in type 2 diabetes mellitus(T2DM)patients by voxel-based morphometry(VBM)and regional homogeneity(ReHo)values.Methods 25 T2DM patients(T2DM group)and 33 normal people(HC group)were enrolled with routine MRI,brain resting state MRI and 3D whole brain high-resolution anatomical image scanning.The ReHo values of the two groups were obtained by processing the resting state image data and the cerebral gray matter volume were obtained by the whole brain high-resolution anatomical images through SPM software.The correlation between the ReHo values and cerebral gray matter volume in all subjects were analyzed.Results Compared with the HC group,ReHo values were reduced in the left hippocampus area,the left fusiform gyrus,the left superior iliac crest,the left caudate nucleus,the right frontal gyrus,the right frontal gyrus,the right anterior cranial anterior lobes,the right parietal lobe and the right thalamusin(P<0.05)and VBM were reduced in the left iliac crest,the left island lobes,the left iliac crest,the right fusiform gyrus,the right lingual gyrus,the right occipital lobe and the right cingulate gyrus T2DM group(P<0.05).Conclusion There is no overlap between brain regions with altered gray matter volume and dysfunctional brain regions in T2DM patients.Changes in gray matter volume and function of T2DM patients may be independent of each other.
