摘 要:目的探讨大螺距低剂量胸部CT扫描在新型冠状病毒肺炎(COVID-19)复查中的应用价值.方法选取经临床诊断疑似及确诊的40例COVID-19并需要复查的患者作为研究对象,随机分为试验组(大螺距低剂量扫描)和对照组(常规剂量扫描),每组20例,对比2种扫描方法的图像质量主观评价、客观评价、扫描时间及辐射剂量评价,所有图像均由3位有经验的放射科医师和技师采用盲法进行独立评分,对所获数据采用SPSS 19.0软件进行统计分析.结果试验组图像质量主观评分3分以上者18例;对照组图像质量主观评分3分以上者20例,2组图像质量主观评分比较差异无统计学意义(P=0.021).试验组与对照组肺平均CT值比较,差异无统计学意义(P>0.05);试验组与对照组平均SNR、CNR比较,均无统计学意义(P>0.05),同时扫描时间、剂量长度乘积(DLP)和有效辐射剂量(ED)2组间均有统计学差异(P<0.05).结论大螺距低剂量胸部CT扫描在COVID-19治疗效果复查中可得到与常规CT扫描同样的检查结果,并能有效降低受检者接受的X线辐射剂量和提高患者检查速度,值得临床推广应用.Objective To explore the application value of larg^pitch low-dose chest CT scan in the review of COVID-19.Methods 40 cases of suspected and confirmed COVID-19 were selected as the research subjects,and they were randomly divided into experimental group(large-pitch low-dose scan)and control group(conventional dose scan).Group of 20 cases,comparing the subjective evaluation,objective evaluation,scanning time and radiation dose evaluation of the two scanning methods,all images were independently scored by three experieneed radiologists and technicians using blind methods,a nd the data obtained were used SPSS 19.0 software performs statistical analysis.Results 18 cases had a subjective score of 3 or more in lhe experimental group;20 cases had a subjective score of 3 or more in the control group.There was no significant difference in the subjective score of image quality between the two groups(P=0.021).There was no statistically significant difference in lung CT value between the experimental group and the control group(P>0.05);the experimental group and the control group had no statistically significant difference in SNR and CNR(P>0.05),and scanning time,close length product(DLP)and effective dose(ED)were statistically different between the two groups(P<0.05).Conclusion The large-pitch low-dose chest CT scan can obtain the same examination results as the conventional CT scan in the review of the treatment effect of COVID-19,and can effectively reduce the X-ray radiation dose received by the examinee and increase the examination speed of the patient.It is worthy of clinical practice promote applications.
