米氮平与舍曲林治疗高血压合并抑郁焦虑的对照研究

摘　　要：目的比较米氮平和舍曲林治疗高血压合并抑郁焦虑患者的临床疗效及安全性。方法将符合纳入标准的70例患者分为两组,在常规降压药物的基础上,分别接受米氮平和舍曲林治疗,采用广泛性焦虑障碍量表(GAD-7)及健康问卷抑郁症状群量表(PHQ-9)评定其抑郁焦虑状况。结果治疗后,米氮平组的收缩压、舒张压均低于舍曲林组,差异显著(P<0.05)。米氮平组总有效率88.6%,舍曲林组总有效率85.7%,两组总有效率相比差异无统计学意义(P>0.05)。治疗后两组GAD-7和PHQ-9评分均较治疗前显著下降(P<0.05),在第8周末两组GAD-7和PHQ-9评分差异均无统计学意义(P>0.05);米氮平组在治疗的第2周末、第4周末的GAD-7和PHQ-9评分较舍曲林组下降更显著(P<0.05)。米氮平组各时段的副反应量表(TESS)评分均低于舍曲林组(P<0.05)。结论米氮平和舍曲林均可改善高血压合并抑郁焦虑患者的症状,疗效显著,但米氮平起效更快,安全性更高。Objective To compare the clinical efficacy and safety of mirtazapine and sertraline for hypertensive patients with anxiety and depression.Methods Totally 70 patients who met the inclusion criteria were divided into two groups.On the basis of conventional antihypertensive drugs,they were treated with mirtazapine and sertraline respectively.GAD-7 and PHQ-9 were used to assess the anxiety and depression in both groups.Results After the treatment,SBP and DBP of the mirtazapine group were lower than those of the sertraline group,with significant difference(P<0.05).The total effective rate was 88.6%in the mirtazapine group and 85.7%in the sertraline group,with no significant difference(P>0.05).After the treatment,GAD-7 and PHQ-9 scores of both groups were lowered more significantly than those of pretreatment(P<0.05).There was no difference at the end of the eighth week between the two groups(P>0.05).At the end of the second week,and fourth week the GAD-7 and PHQ-9 scores were lowered more significantly in the mirtazapine group than in the sertraline group(P<0.05).The TESS score of the mirtazapine group was lower than that of the sertraline group(P<0.05).Conclusion Both mirtazapine and sertraline can improve the symptoms of hypertensive patients with depression and anxiety with significant efficacy,but mirtazapine has a faster onset and is safer.
