经皮椎间孔镜腰椎间盘切除术(PELD)、椎板开窗腰椎间盘切除术治疗腰椎间盘突出症的疗效差异比较

摘　　要：目的:对比分析经皮椎间孔镜腰椎间盘切除术(PTED)、椎板开窗腰椎间盘切除术(FD)对腰椎间盘突出症的治疗价值.方法:观察组行经皮椎间孔镜腰椎间盘切除术治疗,对照组行椎板开窗腰椎间盘切除术治疗.结果:2组手术时间对比P>0.05,观察组术中出血量、术后住院时间少于对照组,数据对比P<0.05;2组术前ODI指数对比P>0.05,术后3个月ODI指数观察组较术前降低幅度优于对照组(P<0.05);2组术前FH、ISH、DID、DFJA、ROM对比均P>0.05,术后1年观察组ROM低于对照组,观察组FH、ISH、DID、DFJA高于对照组(均P<0.05).结论:FD、PTED治疗LDH各有利弊,临床医生应根据患者实际情况综合判断后合理取舍治疗方案.Objective:To compare and analyze the therapeutic value of percutaneous foraminal lumbar discectomy and laminectomy for lumbar disc herniation.Methods:percutaneous foraminal lumbar discectomy was performed in the study group and laminectomy was performed in the control group.Results:the operative time of the two groups was P>0.05,and the intraoperative blood loss and postoperative hospital stay in the study group were less than those in the control group,with P<0.05.Compared with the control group,the reduction of ODI index in the study group 3 months after surgery was better than that in the control group(P<0.05).The comparison of preoperative FH,ISH,DID,DFJA and ROM in the two groups was all P>0.05.The ROM in the study group was lower than that in the control group one year after the operation,while the FH,ISH,DID and DFJA in the study group was higher than that in the control group(all P<0.05).Conclusion:There are advantages and disadvantages in the treatment of lumbar disc herniation with laminectomy and percutaneous foraminal lumbar discectomy.
