带线锚钉与克氏针治疗尺骨冠状突骨折的比较

摘　　要：[目的]比较带线锚钉与克氏针固定尺骨冠状突骨折的临床结果。[方法]回顾性分析本院2015年3月~2019年3月手术治疗的80例尺骨冠状突骨折患者的临床资料,其中,40例采用开放复位锚钉内固定,另外40例采用开放复位克氏钉内固定。比较两组围术期、随访和影像资料。[结果]两组均顺利完成手术,锚钉组手术时间、术中出血量、透视次数、住院时间显著少于克氏针组(P<0.05)。随访12个月以上,锚钉组开始主动活动时间、完全负重活动时间均显著早于克氏针组(P<0.05)。随术后时间推移,两组患者活动时VAS评分显著减少,而肘关节伸屈ROM和Mayor评分显著增加(P<0.05)。相应时间点,锚钉组VAS评分、肘伸屈ROM和Mayo评分均显著优于克氏针组(P<0.05)。影像方面,锚钉组骨折复位质量显著高于克氏针组(P<0.05),至末次随访时,两组患者骨折均愈合。[结论]带线锚钉治疗尺骨冠状突骨折疗效显著优于克氏针固定。[Objective]To compare the clinical outcomes of anchor suture versus Kirschner wire for internal fixation of ulnar coronoid process fractures.[Methods]A retrospective study was conducted on 80 patients who underwent open reduction and internal fixation(ORIF)for ulnar coronoid process fractures from March 2015 to March 2019.Of them,40 patients had ORIF with anchor suture,while the remaining 40 patients received K-wires for internal fixation.The two groups were compared regarding documents of perioperative period,follow-up and radiographs.[Results]All patients in both groups received successful operations,the anchor group consumed significantly less operation time,associated with significantly less intraoperative blood loss,fluoroscopy times and hospital stay than the K-wire group(P<0.05).The follow-up period lasted for more than 12 months.The anchor group returned to elbow active motion and full-weight bearing activity significantly earlier than the K-wire group.The VAS score significantly decreased,whereas the flexion-extension ROM and Mayo score significantly increased over time postoperatively in both groups(P<0.05).The anchor group proved significantly superior to the K-wire group in aforesaid items at all corresponding time points postoperatively(P<0.05).Regarding imaging evaluation,the anchor group also was significantly higher to the K-wire group in quality of fracture reduction after operation(P<0.05).To the latest follow-up,bony healing revealed by images achieved in all patients of both groups.[Conclusion]Internal fixation with anchor suture is superior to that with K-wire for ulnar coronoid process fractures.
