摘  要:[目的]观察蚕丝韧带与骨髓干细胞(BMSCs)制成复合生物材料,重建兔前交叉韧带(ACL)的实验效果,研究其在膝关节内修复ACL的疗效。[方法]新西兰大白兔36只,随机分为两组,于右膝切除原ACL,建立骨隧道,复合组采用蚕丝韧带复合BMSCs移植物重建ACL,而对照组采用单纯蚕丝韧带重建ACL。于术后4、8、12周进行大体形态与组织学观察。[结果]蚕丝韧带交联后呈蜂窝状,增大了蚕丝纤维的表面积,有利于BMSCs的贴壁生长。大体观察方面,随术后时间推移,复合组重建的ACL表面滑膜覆盖逐渐增加,有血管形成,而对照组重建ACL表面滑膜覆盖明显不及复合组。组织学观察方面,复合组重建ACL内纤维组织增生,细胞增殖活跃,明显优于对照组。随术后时间推移,两组移植后的成纤维细胞计数均显著增加(P<0.05),相应时间点复合组成纤维细胞计数显著高于对照组,差异有统计学意义(P<0.05)。[结论]BMSCs在膝关节微环境中可以向成纤维细胞转化,利于蚕丝韧带中纤维组织及血管的再生,促进韧带的修复。[Objective]To explore the outcomes of silk ligament combined with bone marrow stem cells(BMSCs)for anterior cruciate ligament(ACL)reconstruction in rabbit.[Method]A total of 36 New Zealand rabbits were randomly divided into two groups,and underwent surgical treatment to resect the original right ACL,and create bone tunnels on the original ACL insertions.After that,ACL was reconstructed with the silk ligament combined BMSCs in the combined group,while with the silk ligament only in the control group.At 4w,8w and 12w postoperatively,gross and histological observations were conducted to compare the two groups.[Results]After crosslinking with collagen,the silk ligament's surface became rough with increasing the surface area,which facilitated adhesion and growth of BMSCs.In term of gross observation,the synovial coverage on the reconstructed ACL significantly enhanced over time in the combined group with blood vessel formation,which was considerably superior to the control group.In addition,fibro tissue ingrowth revealed by histological observation in the reconstructed ACL also significantly improved over time in both group,which in the combined group was remarkably superior to the control group.As time went postoperatively,the number of fibrocytes in the reconstructed ACL significantly increased in both groups(P<0.05),which in the combined group proved significantly superior to the control group at any corresponding time point(P<0.05).[Conclusion]BMSCs in the intraarticular microenvironment do promote proliferation of fibro tissue and blood vessel in the silk ligament,and enhance the ligament repair.
