进口吸入用乙酰半胱氨酸溶液治疗儿童呼吸道感染危重症的疗效及对免疫功能的影响研究

摘　　要：目的分析进口吸入用乙酰半胱氨酸溶液(商品名:富露施)治疗儿童呼吸道感染危重症的疗效及对免疫功能的影响。方法80例儿童呼吸道感染危重症患儿,按随机数字表法分为对照组和观察组,各40例。对照组给予常规治疗,观察组在常规基础上加用进口吸入用乙酰半胱氨酸溶液治疗。比较两组临床症状消失时间、住院时间、治疗效果、不良反应发生情况及治疗前后炎性细胞因子、免疫功能指标。结果观察组临床症状消失时间、住院时间均短于对照组,差异有统计学意义(P<0.05)。治疗后,两组血清降钙素原(PCT)和超敏C反应蛋白(hs-CRP)水平均低于治疗前,且观察组血清PCT水平(1.71±0.11)μg/L和hs-CRP水平(7.19±1.21)mg/L低于对照组的(4.51±0.21)μg/L、(9.27±1.56)mg/L,差异有统计学意义(P<0.05)。治疗后,两组CD4^+、CD8^+、CD4^+/CD8^+优于治疗前,且观察组优于对照组,差异有统计学意义(P<0.05)。观察组总有效率82.50%高于对照组的62.50%,差异有统计学意义(P<0.05)。结论采用进口吸入用乙酰半胱氨酸溶液治疗儿童呼吸道感染危重症临床疗效显著,可有效改善免疫功能,值得推广。Objective To analyze the efficacy of imported acetylcysteine solution for inhalation(trade name:Fulushi)in the treatment of critically ill children with respiratory infections and its impact on immune function.Methods A total of 80 critically ill children were divided into control group and observation group,with 40 cases in each group.The control group received conventional therapy,and the observation group received imported acetylcysteine solution for inhalation on the basis of conventional therapy.The disappearance time of clinical symptoms,hospitalization time,therapeutic effect,occurrence of adverse reactions,and inflammatory cytokines and immune function indexes before and after treatment were compared between the two groups.Results The disappearance time of clinical symptoms and hospitalization time of the observation group were shorter than those of the control group,and the difference was statistically significant(P<0.05).After treatment,the levels of serum procalcitonin(PCT)and high sensitivity C-reactive protein(hs-CRP)of the two groups were lower than those before treatment,and serum PCT(1.71±0.11)μg/L and hs-CRP(7.19±1.21)mg/L of the observation group were lower than(4.51±0.21)μg/L and(9.27±1.56)mg/L of the control group,and the difference was statistically significant(P<0.05).After treatment,CD4^+,CD8^+and CD4^+/CD8^+of the two groups were better than those before treatment,and the observation group was better than the control group,and the difference was statistically significant(P<0.05).The total effective rate 82.50%of the observation group was higher than 62.50%of the control group,and the difference was statistically significant(P<0.05).Conclusion Imported acetylcysteine solution for inhalation shows remarkable clinical efficacy for critically ill children with respiratory infections,and it can effectively improve the immune function of children.It is worthy of promotion.
