摘 要:目的分析双模式(一侧人工耳蜗+对侧助听器)和双侧人工耳蜗植入术(CI)后听觉及言语康复效果,探讨双耳聆听的优势。方法选取2013—2019年行人工耳蜗植入术的患者30例,其中双模式患者15例,双侧CI患者15例。分别测试双耳、单耳各频率的听阈和单音节、双音节、短句的言语识别率,比较其测试结果。结果双侧CI组各频率的听阈及言语识别率双耳优于单耳(P<0.05),差异有统计学意义。双模式组各频率的听阈和短句识别差异无统计学意义;单音节、双音节的识别双耳(一侧人工耳蜗+对侧助听器)优于单耳(单独使用人工耳蜗)(P<0.05),差异有统计学意义。比较双侧CI组与双模式组双耳听阈和言语识别率差异无统计学意义(P<0.05)。结论双耳双模式和双侧CI都能从言语识别方面体现很好的双耳聆听效果,而两者之间并无显著差别。若双耳无残余听力的耳聋患者,双侧CI能得到更好的双耳听觉;如果一侧耳尚有残余听力或戴助听器后有较大获益,可以优先尝试双耳双模式。Objective To report auditory and speech rehabilitation outcomes in bimodal and bilateral cochlear implantation,and discuss the advantages of binaural listening.Methods Auditory thresholds and the speech recognition scores were compared in 30 patients with cochlear implantation during the period of 2013—2019,including 15 patients with bimodal fitting and 15 patients with bilateral cochlear implants.Results Auditory thresholds at all frequencies but 6000 Hz and speech recognition scores were better with bilateral than with unilateral cochlear implantation usage(P<0.05).There was no significant difference in auditory threshold and short sentence recognition between bilateral and unilateral bimodal fitting.The recognition of monosyllabic and disyllabic words was better with bilateral than with unilateral bimodal fitting(P<0.05),but not significantly different between bimodal fitting and bilateral cochlear implants.Conclusions There was no statistically significant difference between bimodal fitting and bilateral cochlear implantation,although bimodal fitting is preferred,if there is residual hearing or significant benefit in wearing a hearing aid.
