摘 要:目的:探讨神经生长因子(ner vegrowthfactor,NGF)及其低亲和力p75神经营养素受体(p75neurotrophinreceptor,p75NTR)和高亲和力酪氨酸激酶受体A(tyrosinekinaseA,TrkA)在盆腔器官脱垂(pelvic organ prolapse,POP)患者阴道前壁组织中的表达及意义。方法:选择30例因阴道前壁膨出行手术治疗的POP患者,根据POP-Q分度法分为两组,其中14例为POP-Q Ⅱ度,16例为POP-Q Ⅲ~Ⅳ度;同期选择15例非盆底功能障碍性疾病(pelvicfloordysfunction,PFD)患者为对照组。术中取患者阴道前壁相似部位组织,采用RT-PCR和免疫组织化学染色检测NGF,p75NTR和TrkA的表达情况。结果:PCR结果显示对照组患者阴道前壁组织中NGF,p75NTR,TrkA的mRNA表达量分别为1.01±0.06,0.86±0.05,0.95±0.05,POP-Q Ⅱ度组中为0.47±0.07,0.79±0.05,0.43±0.04,POP-QⅢ~Ⅳ度组中为0.41±0.03,0.53±0.04,0.25±0.19,差异有统计学意义(P<0.05)。且POP组p75NTR/TrkA比值较对照组增大。免疫组织化学染色显示POP患者NGF,p75NTR,TrkA的表达水平均降低。结论:NGF及其受体p75NTR和TrkA在POP患者阴道前壁组织中表达减少以及两受体比例的变化,可能与POP的发生、发展相关。Objective: To investigate the expression and significance of nerve growth factor(NGF) and its receptors p75 neurotrophin receptor(p75 NTR) and tyrosine kinase A(TrkA) in the anterior vaginal wall of patients with pelvic organ prolapse(POP). Methods: A total of 30 patients with anterior vaginal wall prolapse were selected and divided into two groups according to POP-Q grading method. Among them, 14 cases were POP-Q Ⅱ degree and 16 cases were POP-Q Ⅲ–Ⅳ degree;15 non pelvic floor dysfunction(PFD) patients were selected as a control group during the same period. Similar parts of the anterior vaginal wall were taken during the operation. The expressions of NGF, p75 NTR and TrkA were detected by RT-PCR and immunohistochemical staining. Results: RT-PCR showed that the mRNA expression levels of NGF, p75 NTR and TrkA in the anterior vaginal wall of the control group were 1.01±0.06, 0.86±0.05, 0.95±0.05, in the group of POP-Q Ⅱ degree were 0.47±0.07, 0.79±0.05, 0.43±0.04 in the group of POP-Q Ⅲ–Ⅳ degree were 0.41±0.03, 0.53±0.04, 0.25±0.19, respectively and the difference was significant(P<0.05). And the ratio of p75 NTR/TrkA in the POP group was higher than that in the control group. Immunohistochemical staining showed that the expression levels of NGF, p75 NTR and TrkA were decreased in POP patients. Conclusion: The decreased expression of NGF and its receptors p75 NTR and TrkA in the anterior vaginal wall of POP patients and the change of the ratio of the two receptors may be related to the occurrence and development of POP.
