联合应用两支血管系统的腹股沟部皮瓣临床观察

摘　　要：目的探讨联合应用两支血管系统的腹股沟部皮瓣治疗手部软组织缺损患者的临床效果。方法选取手部软组织缺损患者60例,皮肤缺损面积:4 cm×4 cm^12 cm×8 cm,采用联合双支主干腹股沟部皮瓣进行修复,通过分析患者术后主观评定、客观评定、并发症发生情况的差异性,探究联合应用两支血管系统的腹股沟部皮瓣的临床效果。结果治疗后,患者对术后外观满意46例,占76.67%,较满意11例,占18.33%;对功能恢复情况满意22例,占36.67%,较满意34例,占56.67%;术后无疼痛感觉53例,占88.33%,未出现持续疼痛情况,患者外观、疼痛及功能恢复情况差异有统计学意义(P<0.05)。采用两支血管系统联合治疗后皮瓣存活率高达100.00%,同时,患者血供质地、感觉恢复情况均较优,且上述因素差异有统计学意义(P<0.05)。治疗后,60例患者出现1例静脉栓塞,1例远端部分坏死,1例皮下淤血,发现异常后,及时进行救治,无切口感染、开裂、腹壁疝膨出等症状出现。结论联合应用两支血管系统的腹股沟部皮瓣治疗手部软组织缺损方案较佳,成功率100%,患者对外观、疼痛情况满意度较高。Objective To study the clinical application of inguinal flap to treat patients with hand soft tissue defect with two vascular systems.Methods 60 patients with hand soft tissue defect were selected.The area of skin defect was from 4cm×4cm to 12cm×8cm to repair by two main groin flap.The differenceS of subjective evaluation,objective evaluation and complications after operation were analyzedto explore the clinical effect of combined application of two vascular system groin flap.Results After treatment,46 patients were satisfied with the appearance of operation site,accounting for 76.67%,11 patients were quite satisfied with the appearance of operation site,accounting for 18.33%;22 patients were satisfied with the function recovery,accounting for 36.67%,34 patients were satisfied with the appearance of operation site,accounting for 56.67%;53 patients had no pain feeling of operation site,accounting for 88.33%,no persistent pain occurred.The differences were significant between appearance,pain and function recovery(P<0.05).The results showed that the survival rate of flap was 100.00%after the combined treatment of two vascular systems.Meanwhile,the quality of blood supply and the recovery of sensation were better,and there were significant differences(P<0.05).After treatment,1 patient of vein embolism,1 patient of distal necrosis and 1 patient of subcutaneous congestion developed among the 60 patients.After being found the abnormality,all patients were treated in time without incision infection,cracking,abdominal hernia and other symptoms.Conclusion The treatment of inguinal flap with two vascular systems treating hand soft tissue defect was better.The success rate was 100%,and the patients were satisfied with the appearance and less pain.
