超声引导下胸椎旁神经阻滞对老年腹腔镜胆囊手术患者应激反应的影响

摘　　要：目的观察探讨超声引导下胸椎旁神经阻滞(thoracic paravertebal blockede,TPVB)用于老年人腹腔镜胆囊切除术的应激反应。方法选择2019年1—12月期间行老年腹腔镜胆囊切除手术患者60例,根据麻醉方法不同分为椎旁阻滞组和对照组各30例,对两组患者血糖和皮质醇浓度、术后VAS评分及不良反应情况进行对比。结果切皮时、术毕时椎旁阻滞组血糖和皮质醇浓度明显低于对照组,术后VAS评分椎旁阻滞组明显低于对照组,椎旁阻滞组中不良反应发生率为10.00%明显低于对照组的46.66%(P<0.05)。结论老年腹腔镜胆囊手术患者应用超声引导下胸椎旁神经阻滞能降低应激反应,减少围麻醉期并发症,减轻术后疼痛。Objective To observe and explore the stress response of TPVB in laparoscopic cholecystectomy for the elderly.Methods 60 elderly patients who underwent laparoscopic cholecystectomy from January to December 2019 were divided into paravertebral block group and control group according to different anesthesia methods.The lood glucose and cortisol concentrations,VAS scores and adverse reactions of the two groups were compared.Results The blood glucose and cortisol concentration in the paravertebral block group were significantly lower than those in the control group.The VAS score of paravertebral block group was significantly lower than that of control group.The incidence of adverse reactions was 10.00%in paravertebral block group,which was significantly lower than 46.66%in control group(P<0.05).Conclusion The application of ultrasoundguided paravertebral nerve block in elderly patients with laparoscopic cholecystectomy can reduce stress response.reduce perioperative complications.and reduce postoperative pain.
