不典型抗精神病药在精神分裂症患者中的应用

摘　　要：目的探究不典型抗精神病药对精神分裂症患者甲状腺功能的影响。方法将我院2017年1月—2019年1月收治的80例精神分裂症患者作为研究组,同时随机抽取80例健康人员为参照组,对比两组甲状腺激素水平。研究组80例患者40例采用奥氮平治疗,另外40例采用利培酮进行治疗,分析不同不典型抗精神病药物对患者甲状腺激素水平的影响。结果(1)研究组与参照组对比:研究组治疗前血浆T3、T4水平明显低于参照组(P<0.05)。(2)研究组患者不同不典型抗精神病药的应用对比:奥氮平治疗前后T3水平对比差异明显(P<0.05);利培酮治疗前后TSH水平对比差异明显(P<0.05);奥氮平、利培酮治疗后T3、TSH水平差异明显(P<0.05)。结论精神分裂症患者可能本身存在甲状腺功能异常。奥氮平治疗精神分裂症对患者T3水平影响较为明显,利培酮治疗精神分裂症对患者TSH水平影响较为明显,应用不典型抗精神病药时医护工作人员应加强对患者甲状腺激素水平的监测。Objective To explore the effect of atypical antipsychotics on thyroid function in patients with schizophrenia.Methods 80 schizophrenic patients in our hospital from January 2017 to January 2019 were randomly selected as the control group,and the thyroid hormone levels of the two groups were compared.In the study group,80 patients(40 cases)were treated with olanzapine and the other 40 cases were treated with risperidone.Results(1)Compared with the control group,the plasma T3 and T4 levels in the study group were significantly lower than those in the reference group before treatment(P < 0.05). (2) Application Comparison of different atypicalantipsychotics in study group: T3 level before and after olanzapinetreatment was significantly different (P < 0.05);TSH level beforeand after risperidone treatment was significantly different (P <0.05);after olanzapine and risperidone treatment, T3 and TSHlevels were significantly different (P < 0.05). Conclusion Patientswith schizophrenia may have thyroid dysfunction. The effect ofolanzapine on the level of T3 was more obvious in the treatment ofschizophrenia, and the effect of risperidone on TSH level was moreobvious. Medical staff should strengthen the monitoring of thyroidhormone level when using atypical antipsychotics.
