输尿管软镜对肾结石患者氧化应激状态的影响及抗炎机制研究

摘　　要：目的研究输尿管软镜对肾结石患者氧化应激状态的影响及抗炎机制并进行分析。方法选取我院2017年2月~2019年8月收治的肾结石患者92例进行研究。根据病情及患者愿望将患者分为软镜组和对照组,对照组实施经皮肾镜术式治疗,软镜组则采用输尿管软镜术式治疗。比较两组手术前后肾功能指标、氧化应激状态指标以及炎症细胞因子水平变化情况,术后并发症发生情况。结果术后24h时软镜组尿KIM-1以及血清CysC、NGAL水平均显著低于对照组(P<0.05)。术后24h时软镜组MDA水平低于对照组,而GSH-Px、SOD水平均高于对照组(P<0.05)。术后24h时软镜组血清TNF-α、IL-6、CRP水平均显著低于对照组(均P<0.05)。软镜组术后并发症总发生率低于对照组(P<0.05)。结论相较于经皮肾镜而言,输尿管软镜对肾结石患者的肾功能、氧化应激状态、炎症反应的影响较小,且安全性更高,值得临床推广应用。Objective To research the impacts of flexible ureteroscope on oxidative stress status and antiinflammatory mechanism in patients with renal calculus.Methods A total of 92 patients with renal calculus admitted to our hospital from February 2017 to August 2019 were selected and divided into the flexible ureteroscope group and the control group according to their conditions and willingness.The control group was treated with percutaneous nephrolithotomy,while the flexible ureteroscope group was treated with flexible ureteroscope.The changes of renal function indexes,oxidative stress status indexes,inflammatory cytokine levels and postoperative complications were compared between the two groups before and after operation.Results 24h after operation,urine KIM-1,serum CysC and NGAL levels in the flexible ureteroscope group were significantly lower than those in the control group(P<0.05).And MDA level in the flexible ureteroscope group was lower than that in the control group,while GSH-Px and SOD levels were higher than those in the control group(P<0.05).The levels of TNF-α,IL-6 and CRP in the flexible ureteroscope group were significantly lower than those in the control group 24 hours after operation(P<0.05).The total incidence rate of postoperative complications in the flexible ureteroscope group was lower than that in the control group(P<0.05).Conclusion Compared with percutaneous nephroscope,flexible ureteroscope has less impacts on renal function,oxidative stress status and inflammatory reaction of renal calculus patients with a higher safety,which is worthy of clinical promotion and application.
