主动脉夹层不同DeBakey分型患者病史及临床特征比较分析

摘　　要：目的研究主动脉夹层不同DeBakey分型患者的相关病史及临床特征,探讨其对疾病发生和预后的影响及其意义。方法纳入兰州大学第一医院2010年1月1日~2017年12月31日主动脉夹层患者593例,以DeBakey分型为分型标准,回顾性分析患者的临床资料及观察患者临床特征。结果高血压患者发生主动脉夹层DeBakeyⅢ型概率高于Ⅰ型和Ⅱ型,差异有统计学意义(P<0.05)。主动脉夹层DeBakeyⅠ型和Ⅲ型患者胸痛和背痛发生率高于Ⅱ型患者,差异有统计学意义(P<0.05)。主动脉夹层DeBakeyⅠ型和Ⅱ型患者胸闷发生率高于Ⅲ型患者,差异有统计学意义(P<0.05)。结论主动脉夹层动脉瘤DeBakeyⅠ型患者的临床症状较其他两型明显,主要表现为胸闷、胸痛及背痛;已确诊为高血压的患者并发主动脉夹层多为DeBakeyⅢ型。Objective To research the related medical history and clinical features of patients with aortic dissection of different DeBakey types,and to investigate its impact on the onset and prognosis of the disease and its significance.Methods A total of 593 patients with aortic dissection were admitted to the First Hospital of Lanzhou University from January 1,2010 to December 31,2017 were included in this research with the DeBakey as the typing standard,and the clinical data of patients were retrospectively analyzed,and the clinical features of patients were observed.Results The probability of DeBakey typeⅢaortic dissection in hypertension patients was higher than that of type I and typeⅡ,and the difference was statistically significant(P<0.05).The incidences of chest pain and back pain in patients with DeBakey type I and typeⅢaortic dissection were higher than those in patients with typeⅡ,and the differences were statistically significant(P<0.05).The incidences of chest distress in patients with DeBakey type I and typeⅡaortic dissection were higher than those in patients with typeⅢ,and the differences were statistically significant(P<0.05).Conclusion The clinical symptoms of patients with DeBakey type I aortic dissection are more obvious than those of the other two types.The main manifestations are chest distress,chest pain and back pain.Most patients with hypertension complicated with aortic dissection are DeBakey typeⅢ.
