体外无创性心脏起搏在院前救治慢性心律失常的应用效果

摘　　要：目的探讨体外无创性心脏起搏(ENCP)在院前救治慢性心律失常的效果。方法将我院急诊科2018年1月~2019年6月院前急救接诊的60例慢性心律失常患者随机分为经静脉临时心脏起搏术(TCP)组与ENCP组。TCP组患者实施经静脉穿刺起搏术,ENCP组行无创心脏起搏,比较两组患者起搏有效性、起搏成功时间及并发症。结果ENCP组与TCP组在慢性心律失常患者救治的起搏成功率、复苏成功率比较,差异无统计学意义(P>0.05)。ENCP组的起搏成功时间显著低于TCP组[(2.13±0.53)min vs(.5.22±1.36)min](P<0.01)。ENCP组的并发症总发生率显著低于TCP组(10.00%vs.46.67%)(P<0.01)。结论ENCP与TCP在院前救治慢性心律失常患者效果相当,但ENCP可显著缩短起搏成功时间,降低心脏起搏相关并发症。Objective To investigate the efficacy of external non-invasive cardiac pacing(ENCP)in pre-hospital treatment of chronic arrhythmia.Methods A total of 60 patients with chronic arrhythmia admitted to our hospital through pre-hospital first aid of Department of Emergency from January 2018 to June 2019 were randomly divided into the transvenous temporary cardiac pacing(TCP)group and the ENCP group.The patients in the TCP group received venipuncture pacing while the patients in the ENCP group received non-invasive cardiac pacing.The effectiveness,success time and complications of pacing in the two groups were compared.Results There was no statistically significant difference in pacing success rate and resuscitation success rate between the ENCP group and the TCP group in treating patients with chronic arrhythmia(P>0.05).The pacing success time([2.13±0.53]min vs.[5.22±1.36]min)in the ENCP group was significantly lower than that in the TCP group(P<0.01).The overall incidence of complications(10.00%vs.46.67%)in the ENCP group was significantly lower than that in the TCP group(P<0.01).Conclusion The efficacy of ENCP and TCP in pre-hospital treatment of patients with chronic arrhythmia is equivalent,but ENCP can significantly shorten the pacing success time and reduce complications related to cardiac pacing.
