摘 要:目的探讨3D-DESS序列与常规SE序列T2WI-TSE-FS序列、PDWI-TSE-FS序列评估血友病性关节病骨软骨病变的价值。方法采用西门子SKYRA 3.0T MRI对27个关节(膝、踝和肘)行磁共振扫描,M R I序列包括三维双回波稳态成像(three dimensional dual-echo steady state,3D-DESS)、T2加权像快速自旋回波脂肪抑制(T2WI-TSE-FS)、质子加权脂肪抑制快速自旋回波(PDWIT SE-FS)。应用国际血友病预防治疗研究组(International Prophylaxis Study Group,IPSG)评分表,分别对关节软骨损伤、骨表面侵蚀、小囊变进行评分,比较这三个序列的评分在评估血友病性关节病骨软骨病变的结果的差异,P<0.05具有统计学意义。结果3D-DESS、T2WI-TSE-FS、PDWI-TSEFS序列显示关节软骨病变的IPSG评分分别为(2.00±1.24)、(0.48±0.75)、(0.74±0.94),3D-DESS序列关节软骨病变IPSG评分明显高于常规SE序列,差异具有统计学意义;3D-DESS、T2WI-TSEFS、PDWI-TSE-FS序列骨表面侵蚀的IPSG评分分别(0.89±0.75)、(1.37±0.69)、(1.33±0.68),常规SE序列的骨表面侵蚀IPSG评分明显高于3D-DESS序列,差异具有统计学意义;三个序列的小囊变IPSG评分差异不具有统计学意义。结论3D-DESS序列较T2WI-TSE-FS、PDWI-TSE-FS显示关节软骨更清晰,是显示关节软骨病变的最佳扫描序列之一,对于诊断血友病性关节病的关节软骨病变有较高的临床应用价值。Objective To evaluate the value of 3D-DESS sequence and conventional SE sequence T2WI-TSE-FS sequence and PDWI-TSE-FS sequence in evaluating bone cartilage lesions of hemophiliac arthropathy.Methods Scanned 27 joints(knee,ankle and elbow)with Siemens SKYRA 3.0T MRI 3D-DESS,T2WI-TSE-FS,PDWI-TSE-FS sequences.Application of the International Prophylaxis Study Group(IPSG)score sheet,cartilage degradation,surface erosions involving subchondral cortex or joint margins and subchondral cysts were scored respectively.Comparing the scores of these three sequences in evaluating osteochondral lesions in hemophiliac arthropathy,P<0.05 is statistically significant.Result The IPSG scores of 3D-DESS,T2WI-TSE-FS and PDWI-TSEFS articular cartilage lesions were(2.00+1.24),(0.48+0.75)and(0.74+0.94),respectively.The IPSG score of 3D-DESS sequence articular cartilage lesions was significantly higher than that of conventional sequences,and the difference was statistically significant.The IPSG scores of 3D-DESS,T2WI-TSE-FS and PDWI-TSE-FS sequences of bone surface erosion were(0.89±0.75),(1.37±0.69),(1.33±0.68),respectively.The IPSG score of bone surface erosion in conventional SE sequences was significantly higher than that of 3D-DESS sequence,and the difference was statistically significant.There was no significant difference in IPSG scores among three sequences of subchondral cysts.Conclusion The 3D-DESS sequence shows clearer articular cartilage than T2WI-TSEFS and PDWI-TSE-FS,and is one of the best scanning sequences for articular cartilage lesions.It has high clinical value in the diagnosis of articular cartilage lesions in hemophiliac arthropathy.
