摘 要:目的探讨脑卒中并发肺部感染的危险因素。方法选择2018年6月至2019年7月在我院接受治疗的脑卒中患者84例为研究对象,根据是否出现肺部感染分为未发生肺部感染62例和发生肺部感染22例,采用多因素logistic回归分析脑卒中并发肺部感染的危险因素。结果脑卒中并发肺部感染患者共培养分离病原菌56株,其中鲍曼不动杆菌37株、产超光谱β-内酰胺酶5株、肠杆菌科细菌6株、金黄色葡萄球菌4株、铜绿假单胞菌4株。多因素logistic回归分析显示,意识障碍、气管切开、机械通气、手术是影响脑卒中并发肺部感染的独立危险因素(P<0.05)。结论脑卒中患者并发肺部感染的危险因素较多,临床治疗应重视增强患者免疫功能以及规范应用抗菌药物。Objective To explore the risk factors of stroke complicated with lung infection.Methods To select 84 cases of stroke patients who were treated in our hospital from June 2018 to July 2019 was set as the study objects,and according to whether there was lung infection,they were divided into 62 cases without lung infection and 22 cases with lung infection.Multivariate logistic regression analysis was used to analyze the risk factors for stroke complicated with lung infection.Results There were 56 pathogenic bacteria co-cultured and isolated from patients with stroke and lung infection,including 37 Acinetobacter baumannii,5 hyperspectralβ-lactamase,6 Enterobacteriaceae bacteria,4 Staphylococcus aureus,4 Pseudomonas aeruginosa.Multivariate logistic regression analysis showed that the disturbance of consciousness,tracheotomy,mechanical ventilation and operation were the independent risk factors that affected stroke and lung infection(P<0.05).Conclusion There were many risk factors affecting lung infection in stroke patients.Clinical treatment should pay attention to enhancing the immune function of patients and standardizing the application of antibacterial drugs.
