摘 要:目的分析变态反应性疾病患者食物特异性IgG抗体水平,探讨其与食物种类、年龄、性别的关系,为临床诊治提供参考。方法应用酶联免疫吸附测定(ELISA)法检测2018年12月~2019年12月在新疆维吾尔自治区人民医院皮肤科就诊的1098例变态反应性疾病患者血清总IgE与14种食物特异性IgG抗体水平。结果1098例变态反应性疾病患者中检出血清总IgE阳性者638例,阳性率为58.11%;790例IgG抗体阳性者,阳性率为71.95%,其中以鸡蛋、牛奶、小麦、西红柿阳性率较高,以猪肉、牛肉、鸡肉、虾阳性率较低。4类变态反应性疾病血清总IgE阳性率、食物特异性IgG抗体阳性率比较,差异有统计学意义(P<0.05),其中血清总IgE阳性率及食物特异性IgG抗体阳性率均以湿疹皮炎类患者最高。不同性别、民族及年龄的变态反应性疾病患者均以鸡蛋、牛奶、小麦和西红柿检出率较高,关于牛奶、西红柿特异性IgG抗体检出率中男性高于女性,汉族高于少数民族,幼年组和少年组高于青年组、中年组和老年组,差异有统计学意义(P<0.05)。结论食物不耐受的发病与患病种类、性别、民族和年龄的分布有关,血清总IgE和食物特异性IgG抗体均参与变态反应性疾病的发生发展,但参与机制可能不同,及时明确不耐受食物及血清总IgE水平,对临床相关疾病的诊治和预防具有一定的价值。Objective To analyze the level of food-specific IgG antibody in patients with allergic diseases,and to explore the relationship between it and the type of food,age,and gender,so as to provide references for clinical diagnosis and treatment.Methods The enzyme-linked immunosorbent assay(ELISA)method was used to detect the serum levels of total IgE and 14 food-specific IgG antibodies in 1,098 patients with allergic diseases who were treated in the Dermatology Department of People's Hospital of Xinjiang Uygur Autonomous Region from December 2018 to December 2019.Results Among 1098 patients with allergic diseases,638 cases were positive for serum total IgE,with a positive rate of 58.11%;790 cases with IgG antibody positive,with a positive rate of 71.95%,of which eggs,milk,wheat,and tomatoes had higher positive rates,the positive rate of pork,beef,chicken and shrimp is low.Comparison of the positive rate of serum total IgE and food-specific IgG antibody positive rate for 4 types of allergic diseases was statistically significant(P<0.05).Among them,the positive rate of serum total IgE and food-specific IgG antibody positive rate were both eczema dermatitis the highest category of patients.The detection rate of allergic disease patients of different gender,ethnicity and age was higher in eggs,milk,wheat and tomatoes.With regard to the detection rate of milk and tomato-specific IgG antibodies,the detection rate of males was higher than that of females,and the Han nationality was higher than that of ethnic minorities.The young group and the juvenile group were higher than the young group,the middle-aged group and the old group,the difference was statistically significant(P<0.05).Conclusion The onset of food intolerance was related to the distribution of disease type,gender,ethnicity and age.Serum total IgE and food-specific IgG antibodies were involved in the occurrence and development of allergic diseases,but the participating mechanisms might be different,so the intolerance could be clarified in time the total IgE level
