不同抗高血压药对高血压患者动脉僵硬度的影响

摘　　要：目的评价五种不同类型降压药对原发性高血压患者动脉僵硬度的影响。方法本研究中纳入2017年1月~2019年10月于本院就医的150例男性原发性高血压患者,根据服用降血压药物的不同分为五组,每组各30例。培哚普利组患者服用培哚普利片,倍他乐克组患者服用美托洛尔缓释片,氨氯地平组患者服用苯磺酸氨氯地平片,缬沙坦组患者服用缬沙坦胶囊,吲达帕胺组患者服用吲达帕胺,每组疗程均为3个月。分析比较各组治疗前后患者血压变化和动脉僵硬度变化。结果经治疗后,各组患者收缩压(SBP)和舒张压(DPB)均显著下降(P<0.001)。培哚普利、缬沙坦、氨氯地平和吲达帕胺均显著降低了高血压患者的肱-踝动脉脉搏波速度(Ba-PWV)(P<0.001),并显著提高了高血压患者的踝肱指数(ABI)(P<0.001),效果顺次减弱。倍他乐克无显著缓解动脉僵硬度效果。结论在达到相近的降血压效果的情况下,培哚普利缓解高血压患者的动脉僵硬度作用效果最为显著,其他依次为缬沙坦、氨氯地平和吲达帕胺,倍他乐克无明显效果。Objective To evaluate the effects of five different types of antihypertensive drugs on arterial stiffness in patients with essential hypertension.Methods In this study,a total of 150 male patients with essential hypertension,who were treated in our hospital from January 2017 to October 2019,were divided into five groups according to taking different antihypertensive drugs,with 30 people in each group.Patients in the perindopril group received perindopril tablets;patients in the betadock group received metoprolol sustained-release tablets;patients in the amlodipine group received amlodipine besylate tablets;Patients in the valsartan group took valsartan capsules;patients in the indapamide group took indapamide.The course of treatment was 3 months.Analysis and comparison of blood pressure and arterial stiffness before and after treatment in each group.Results After treatment,the Systolic Blood Pressure(SBP)and Diastole pressure(DPB)of each group of patients decreased significantly(P<0.001).Perindopril,valsartan,amlodipine and indapamide significantly reduced the brachial-ankle artery pulse wave velocity(Ba-PWV)in hypertensive patients(P<0.001),and significantly increased Ankle brachial index(ABI)in patients with hypertension(P<0.001).The effect of perindopril was the best,and the effects of valsartan,amlodipine,and indapamide were gradually weakened.Conclusion Under the similar blood pressure lowering effect,perindopril has the most significant effect in alleviating arterial stiffness in patients with hypertension,followed by valsartan,and the rest are amlodipine and indapamide,and betaxol obviously protect blood vessels.
