日间手术实施分散式与集中式联合管理模式对床位周转率及医疗费用支出的影响

摘　　要：目的探讨日间手术实施分散式与集中式联合管理模式对床位周转率及医疗费用支出的影响。方法回顾性选取2018年1月~2020年1月我院日间手术患者100例,依据管理模式分为分散式与集中式联合管理模式组(联合管理模式组,n=50)和单独分散式管理模式组(单独管理模式组,n=50)两组,统计分析两组的出院例数、手术例数、单位床位手术例数、平均住院时间、医疗费用支出。结果联合管理模式组出院例数为(28.30±3.20)例/月,显著多于单独管理模式组的(14.20±3.30)例/月(P<0.05),手术例数为(0.93±0.11)床/月,显著多于单独管理模式组的(0.75±0.10)床/月(P<0.05),单位床位手术例数为(19.40±3.00)例/床,显著多于单独管理模式组的(13.90±2.70)例/床(P<0.05)。联合管理模式组患者的平均住院时间为(10.70±1.90)d,显著短于单独管理模式组的(12.70±2.00)d(P<0.05),医疗费用支出为(11888.20±2066.40)元,显著低于单独管理模式组的(14007.20±2020.30)元(P<0.05)。在应激并发症总发生率方面,联合管理模式组为12.0%(6/50),单独管理模式组为22.0%(11/50),前者显著低于后者(P<0.05)。结论日间手术实施分散式与集中式联合管理模式较单独分散式管理模式更能有效提升床位周转率,降低患者医疗费用支出。Objective To investigate the impacts of decentralized and centralized combined management mode on bed turnover rate and medical expenses in day surgery.Methods From January 2018 to January 2020,a total of 100 patients treated by day surgery admitted to our hospital were retrospectively selected and divided into two groups according to the management mode.They were the decentralized and centralized combined management mode group(the combined management mode group,n=50)and the single decentralized management mode group(single management mode group,n=50).The numbers of discharged cases,operation cases,operation cases per bed,average hospitalization time and medical expenses of the two groups were statistically analyzed.Results The number of discharged cases was(28.30±3.20)cases per month in the combined management mode group,which was significantly higher than(14.20±3.30)cases per month in the single management mode group(P<0.05).And the number of operation cases(0.93±0.11)bed per month in the combined management mode group was significantly higher than(0.75±0.10)bed per month in the single management mode group.The number of operation cases per bed in the combined management mode group was(19.40±3.00),which was significantly higher than(13.90±2.70)in the single management mode group(P<0.05).The average hospitalization time in the combined management mode group was(10.70±1.90)d,which was significantly shorter than(12.70±2.00)d in the single management mode group(P<0.05).Meanwhile,the medical expenses were(11888.20±2066.40)CNY,which was significantly lower than(14007.20±2020.30)CNY in the single management mode group(P<0.05).The total total incidence of stress complications was 12.0%(6/50)in the combined management mode group,which was significantly lower than 22.0%(11/50)in the single management mode group(P<0.05).Conclusion Compared with the single decentralized management mode,the decentralized and centralized combined management mode effectively improves the bed turnover rate and reduces the patient’s
