摘　　要：目的:比较痔上黏膜环切术(PPH)联合外剥内扎加内括约肌侧切术与PPH加外剥内扎术治疗环状混合痔的效果。方法:选取环形混合痔患者80例,按照随机数字表法,分为对照组与观察组各40例。对照组治疗方法为PPH联合外剥内扎,观察组在对照组基础上加用内括约肌侧切术治疗,比较两组手术指标与术后3d、7d的伤口疼痛评分、水肿评分和出血评分。结果:两组手术时间差异无统计学意义(P>0.05);相较于对照组,观察组术后创面愈合时间与住院时间更短(P<0.05);相较于对照组,观察组术后3d、7d的伤口疼痛评分、水肿评分和出血评分均更低(P<0.05)。结论:PPH联合外剥内扎加内括约肌侧切术治疗环状混合痔的疗效确切,可减轻患者伤后疼痛、水肿与出血症状,并可加快术后创面愈合速度。Objective:To compare the effect of PPH combined with external dissection and internal ligation plus lateral sphincter resection in the treatment of circumferential mixed hemorrhoids.Methods:80 cases of circular mixed hemorrhoids were randomly divided into control group and observation group,40 cases in each group.The control group was treated with PPH combined with external dissection and internal ligation,while the observation group was treated with internal sphincter lateral incision on the basis of the control group.The operation indexes and wound pain score,edema score and bleeding score of the two groups at 3D and 7d after operation were compared.Results:There was no significant difference in the operation time between the two groups(P>0.05);compared with the control group,the wound healing time and hospitalization time of the observation group were shorter(P<0.05);compared with the control group,the wound pain score,edema score and bleeding score of the observation group were lower at 3D and 7d after operation(P<0.05).Conclusion:PPH combined with external dissection and internal ligation plus internal sphincter lateral resection is effective in the treatment of circumferential mixed hemorrhoids,which can reduce the pain,edema and bleeding symptoms of patients after injury,and accelerate the wound healing speed after operation.
