供精人工授精妊娠结局的影响因素分析

摘　　要：目的探讨影响供精人工授精(AID)妊娠结局的临床影响因素。方法回顾性分析2015年至2018年在福建省妇幼保健院生殖中心接受AID治疗的病例660例,共1305个AID周期,探索各临床影响因素,分析女方年龄、不孕年限、体重指数(BMI)、AID日内膜厚度和授精方式等与AID妊娠结局的关系。结果单因素分析显示,女性AID治疗的<25岁组的妊娠率(31.7%)高于25~35岁组(22.8%),≥35岁妊娠率(19.4%)降低更明显;不孕年限≤5年组妊娠率(24.8%)高于>5年组(18.6%);宫腔内人工授精组(IUI)的妊娠率(26.2%)高于宫颈管内人工授精组(ICI,15.6%);多因素logistic回归分析显示,女方年龄和授精方式是AID妊娠率的主要影响因素。结论AID的妊娠率随女方年龄增大而降低;女方年龄小者、采用IUI法者,AID的妊娠率较高。Objective To explore the clinical influencing factors of the pregnancy outcomes in artificial insemination by donor(AID).Methods A retrospective study was performed to analyze the pregnancy outcomes of AID for 660infertile couples in 1305treatment cycles from 2015to 2018in Fujian Provincial Maternity and Child Health Hospital.The relationship between influencing factors(maternal age,duration of infertility,BMI,endometrial thickness and location of insemination)and AID outcomes was analyzed.Results Single factor analysis showed that pregnancy rate of maternal aged≤25(31.7%)was higher than that of aged 2535(22.8%),the aged≥35(19.4%)decreased more obviously.The pregnancy rate of maternal infertility duration≤5years(24.8%)was higher than that of>5years(18.6%).The pregnancy rate in intrauterine insemination(IUI)group(26.2%)was high than that of intracervical insemination(ICI)group(15.6%).The maternal age and location of insemination were the key influencing factors of pregnancy rate of AID by multivariate logistic regression analysis.Conclusion The AID pregnancy rates of AID decreased with the maternal ages,pregnancy rate of AID was higher for young woman and the method with IUI.
