摘　　要：目的探讨医护联合门诊在提高非瓣膜病房颤患者华法林治疗依从性中的作用。方法选择2018年7月—2019年6月本院心血管内科门诊就诊的128例非瓣膜病房颤患者,按照门诊号采用随机数字表法分为对照组与试验组,每组各64例,对照组患者进行单纯医生门诊治疗及管理,试验组患者采用医护联合门诊干预,1名医生和1名护士共用1个诊室联合门诊,对患者进行诊治宣教以及定期随访,干预时间12个月。比较两组患者12个月后抗凝治疗依从性、出血和栓塞发生率和患者满意度。结果干预后试验组患者服药饮食控制和复诊检查等维度及总治疗依从性评分均高于对照组;试验组患者栓塞以及出血的不良反应发生率低于对照组,试验组患者满意度高于对照组,两组比较,差异有统计学意义(均P<0.05)。结论非瓣膜性房颤门诊患者华法林抗凝治疗采用医护联合门诊,可提高抗凝治疗患者依从性,减少出血和栓塞的发生率,患者满意度较高,值得临床推广应用。Objective To study the effect of doctor-nursing joint clinical consultations on Warfarin treatment compliance compliance of patients with non-valvular atrial fibrillation.Methods Totally,128 patients with non-valvular atrial fibrillation visiting for medical consultations in Department of Cardiovascular Diseases during July 2018 and June 2019 were randomized into control and trial groups in equal number by using a random digit table:the former group was treated with doctor's consultations alone and the latter with doctor-nursing joint consultations on diagnosis and treatment as well as joint education,followed by regular follow-ups.The two groups were compared in terms of anticoagulation compliance,the rate of bleeding and embolism and the comfort degree.Results The medication and diet control,follow-up examination in the trial group were significantly better than the control group(P<0.05)and the total score of Warfarin treatment compliance was significantly higher than the control group(P<0.05).The rates of bleeding and embolism of the trial group were significantly lower than the control group(P<0.05).Conclusion The mode of doctor-nursing joint clinical consultations can enhance Warfarin treatment compliance of patients with non-valvular atrial fibrillation,reduce the rate of bleeding and embolism and improve the comfort degree of patients,worth clinical spreading.
