摘 要:本文在介绍我院个案管理门诊构建背景及基础上,对其运作流程相关的收案、管案、结案、信息系统建立进行全面报告。个案管理门诊通过多学科整合、联合性照护,在提高患者对医院服务的满意度、自我管理能力以及依从性方面有显著成效;能够为慢性病患者提供更全面的治疗与照顾,改善生活品质、同时拓展了护理的功能和范围,为护士的职业发展提供了更多的方向,值得在慢性病管理中借鉴并推展。Based on the background introduction and construction of the case management clinic in our hospital,this paper focuses on case collection,case management,closure and information system establishment related to its operation process.Through multidisciplinary integration and joint care,case management clinic service has significant effects in improving patients'satisfaction of hospital services and their self-management ability and compliance.It can provide more comprehensive treatment and care for patients with chronic diseases,improve their qualities of life,reduce medical costs,as well as expand the functions and scope of nursing,which will provide more direction for career development of nursing staff.Thus it is worth learning and promoting in chronic disease management.
