抗凝护理专科门诊在规范肺栓塞患者治疗中的临床实践

摘　　要：肺栓塞患者规范化的抗凝治疗是一项复杂、系统、细致的工作,关乎到患者疾病的转归,开设抗凝护理专科门诊可为患者提供规范化的用药管理。我院于2016年3月建立医护协作型抗凝护理专科门诊,以护士为主导规范抗凝管理,取得了良好成效,提高了抗凝治疗患者的依从性及国际标准化比值达标率,减少了出血事件的发生。本文介绍抗凝护理专科门诊的建立与实施,为护士规范肺栓塞患者的抗凝管理提供借鉴。Standard anticoagulation treatment of pulmonary embolism,which is closely related with the outcomes of patients,is a complex,systematic and meticulous work.The establishment of a nurse-led anticoagulant clinic can provide criteria of anticoagulant treatment for patients.Ever since the set up of the anticoagulant clinic led by specialist nurses in March 2016,pretty good results have been achieved including improved patients'compliance,better international normalized ratio level and reduced rate of bleeding events.This article introduces the establishment and implementation of the nurse-led anticoagulant clinic to share our experience of standard anticoagulant treatment for patients with pulmonary embolism.
