清肺化痰活血方辅助治疗儿童重症支原体肺炎毒热闭肺证随机单盲临床研究

摘　　要：目的评价清肺化痰活血方辅助治疗儿童重症支原体肺炎毒热闭肺证的临床疗效。方法纳入重症支原体肺炎毒热闭肺证患儿78例,随机分为对照组、治疗组各39例。对照组采用西医基础治疗加中药麻杏石甘汤,治疗组采用西医基础治疗加清肺化痰活血方,两组均每日1剂。治疗10天后对比两组临床症状、体征评分、凝血功能及炎症指标的变化,并进行疗效评价。结果治疗组脱落4例,对照组脱落6例,最终纳入治疗组35例,对照组33例。治疗后治疗组患儿痊显率为74.28%,明显高于对照组的51.51%(P<0.05);治疗后两组患者症状、体征评分较治疗前均明显降低(P<0.05),且治疗组咳嗽、胸片X线指征、肺部体征评分明显低于对照组(P<0.05);治疗后两组患儿凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、D-二聚体(D-D)、降钙素原(PCT)、C-反应蛋白(CRP)均较治疗前明显下降(P<0.05),且治疗后治疗组PT、D-D明显低于对照组(P<0.05)。结论清肺化痰活血方辅助治疗重症支原体肺炎毒热闭肺证疗效确切,可显著改善患儿临床症状、体征及凝血功能。Objective To evaluate the clinical efficacy of Qingfei Huatan Huoxue Formula(清肺化痰活血方)for severe mycoplasma pneumonia of toxin-heat blocking lung syndrome in children.Methods A total of 78 patients with severe mycoplasma pneumonia of toxin-heat blocking lung syndrome were randomly divided into a control group and a treatment group,with 39 cases in each group.The control group was treated with basic western medicine plus Maxing Shigan Decoction(麻杏石甘汤),while the treatment group was given Qingfei Huatan Huoxue Formula plus basic western medicine.Both groups were given 1 dose per day for 10 days.Clinical symptoms,physical signs scores,changes in coagulation function and inflammation indicators before and after treatment were compared between groups,and curative effect was evaluated.Results There were 4 cases in the treatment group and 6 cases in the control group falling off.A total of 35 cases in the treatment group and 33 cases in the control group were enventually included.After treatment,the recovery rate of the treatment group was 74.28%,which was significantly higher than 51.51% of the control group(P<0.05).After treatment,the symptoms and signs of both groups were significantly lower than before treatment(P<0.05),and the scores of cough,chest X-ray indications and lung signs in the treatment group were significantly lower than those in the control group(P<0.05);after treatment,prothrombin time(PT),activated partial thromboplastin time(APTT),D-Dimer(D-D),procalcitonin(PCT),C-reactive protein(CRP)of both groups were significantly decreased than those before treatment(P<0.05),and PT and D-D in the treatment group were significantly lower than those in the control group after treatment(P<0.05).Conclusion Qingfei Huatan Huoxue Formula as adjuvent therapy has shown its definite efficacy in treating children severe mycoplasma pneumonia by improving clinical symptoms,signs and coagulation function.
