医护患一体化护理模式在Ⅲ型先天性食管闭锁患者中的应用效果评价

摘　　要：目的探讨医护患一体化护理模式在Ⅲ型先天性食管闭锁(CEA)患者诊治中的临床效果。方法将2017年1月-2020年4月本院就诊的120例CEA患儿遵照随机分配方式分为对照组和观察组,每组各60例。其中对照组给予常规护理,观察组采用医护患一体化护理模式,即在对照组基础上借助微信平台完成住院护理和出院后延续的一体化护理任务。比较两组患儿再次住院率、疑似术后并发症患儿1 h就诊率和准确率、患儿父母的心理状况、就医依从性和满意度。结果观察组患儿随访期间再次住院率低于对照组(P<0.05);观察组的疑似术后并发症患儿1 h就诊率和准确率均高于对照组(P<0.05);观察组患儿家长出院时焦虑评分、抑郁评分均低于对照组(P<0.05);观察组患儿家长的就医依从性和满意度均高于对照组(P<0.05)。结论医护患一体化护理模式用于CEA的临床效果好,减少再住院率,及时诊疗可疑并发症,有效改善父母的负性心理情绪,提高依从性和满意度,值得推广。Objective To evaluate the effective of applying integrated doctor-nurse-patient management model on patients with type III Congenital Esophageal Atresia. Methods A total of children with type Ⅲ Congenital Esophageal Atresia from January 2017 to April 2020 were randomly divided into control group and observation group, with 60 patients in each group. The control group was given routine nursing, while the observation group was given integrated doctor-nurse-patient management model, that is, on the basis of routine nursing, the observation group was applied WeChat platform to complete the integrated nursing tasks not only in-patient nursing care but also the continuous nursing direction for the children family. According to the patients prognosis, rate of re-hospitalization, rate of receiving medical treatment in 1 hour for suspected postoperative complications cases and rate of accuracy for suspected postoperative complications were compared between the two groups. For the children′ parents, the psychological status evaluated by Self-rating Depression Scale(SDS) and Self-rating Anxiety Scale(SAS), medical compliance and satisfaction of the two groups were compared. Results The rate of re-hospitalization in the observation group was lower than that of control group(P<0.05);The rate of receiving medical treatment in 1 hour for suspected postoperative complications cases and rate of accuracy for suspected postoperative complications in the observation group was higher than that of control group(P<0.05);The SDS and SAS scores of parents in the observation group were both lower than those in the control group(P<0.05). The medical compliance and satisfaction of parents in the observation group were higher than those in the control group(P<0.05). Conclusion The integrated doctor-nurse-patient management model for children with CEA has a reliable clinical effect, reduces the rate of re-hospitalization, timely diagnosis and treatment suspicious complications, effectively improves the negative psychological state of pare
