摘 要:目的研究维生素D2对老年肱骨近端骨质疏松性骨折术后的疗效影响。方法纳入老年肱骨近端骨质疏松性骨折患者52例,随机分为实验组(n=27)、对照组(n=25)。实验组行锁定钢板内固定术的同时连续肌肉注射维生素D2 40万单位/日,共七日,对照组仅行锁定钢板内固定术。对比两组术后血清25OHD水平、骨折愈合时间、肩关节Neer功能评分,分析锁定钢板内固定术结合维生素D2在老年肱骨近端骨质疏松性骨折患者中的疗效。结果术后随访6个月时实验组患者肩关节Neer功能评分显著高于对照组(P <0.01),优良率(P <0.05)及并发症发生率均低于对照组,实验组术后骨折愈合时间明显低于对照组(P <0.01),实验组血清25OHD水平在6周、12周、24周时均显著高于对照组(P <0.01)。结论维生素D2能有效缩短老年肱骨近端骨质疏松性骨折患者术后骨折愈合时间,改善患者肩关节功能。Objective To study the efficacy of Vitamin D2 on postoperative osteoporotic proximal humeral fracture in elderly patients. Methods 52 elderly patients with proximal humeral osteoporotic fracture were randomly divided into the experimental group(n = 27)and the control group(n = 25). The patients in the experimental group received vitamin D2 400000 IU everyday by intramuscular after the locking plate fixation, sustaining 7 days;the patients in the control group was just treated with locking plate internal fixation. The serum 25 OHD level, fracture healing time and shoulder Neer function score were compared between the two groups, and the curative effect of locking plate internal fixation combined with vitamin D2 in elderly patients with proximal humeral osteoporotic fracture was analyzed. Results After the follow up for 6 months,the shoulder joint Neer functional score in experimental group was significantly better than that in the control group(P < 0.01),excellent rate(P <0.05) and the complications rate was lower than that of the control group,the fracture healing time in experimental group was significantly lower than that of the control group(P < 0.01), and the serum 25 OHD level on 6 weeks,12 weeks and 24 weeks were significantly higher than that of the patients in the control group(P < 0.01).Conclusion Vitamin D2 can effectively shorten the fracture healing time on postoperative osteoporotic proximal humeral fracture in senior patients,and improve their shoulder joint function.
