摘　　要：目的:研究原发性干燥综合征(pSS)患者血清中高密度脂蛋白(HDL)的水平与炎症免疫指标的相关性,并探讨HDL在干燥综合征疾病进程中的作用。方法:回顾性分析350例pSS患者及350例健康对照者血清中血脂水平及炎症免疫指标,采用ELISA法检测30例pSS患者及30例健康对照者血清中的IL-6和TNF-α水平,分析HDL与炎症免疫指标及炎性细胞因子的相关性。结果:与对照组相比,pSS患者血清中HDL、低密度脂蛋白(LDL)、载脂蛋白AI(ApoAI)及胆固醇的水平均显著降低(P<0.0001)。HDL水平与红细胞沉降率(ESR)(r=-0.38,P<0.0001)及IgG(r=-0.35,P<0.0001)之间均具有有显著的相关性,而LDL水平与各炎症指标及免疫指标均无明显的相关性。pSS患者血清中IL-6及TNF-α水平显著高于对照组,并且pSS患者血清HDL水平与IL-6及TNF-α的水平呈显著的负相关。体外细胞实验表明HDL能以剂量依赖的方式抑制IL-6及TNF-α的产生,而LDL无显著抑制作用。结论:pSS患者血清中HDL水平显著降低,HDL的水平与pSS患者炎症水平呈显著负相关。Objective:To study the correlation between the level of high-density lipoprotein(HDL)in serum of patients with primary Sjogren’s syndrome(pSS)and inflammation indicators,and to explore the role of HDL in the process of Sjogren’s syndrome.Methods:To retrospectively analyze the serum lipid levels and inflammatory immune indexes in 350 patients with pSS and 350 healthy controls,using ELISA method to detect the serum levels of IL-6 and TNF-α in 30 patients with pSS and 30 healthy controls,and analyze the correlation between HDL and inflammation indicators,immune indicators and inflammatory cytokines.ResμLts:Compared with the control group,the levels of HDL,low-density lipoprotein(LDL),apolipoprotein AI(APoAI)and cholesterol in the serum of pSS patients were significantly reduced(P<0.0001).The level of HDL was significantly correlated with erythrocyte sedimentation rate(ESR)(r=0.38,P<0.0001)and IgG(r=0.35,P<0.0001),while the level of LDL was not significantly correlated with inflammation and immune indexes.The serum levels of IL-6 and TNF-α in pSS patients were significantly higher than those in the control group,the serum HDL levels of pSS patients were significantly negatively correlated with the levels of IL-6 and TNF-α.In vitro cell experiments showed that HDL can inhibit the production of IL-6 and TNF-αin a dose-dependent manner,while LDL has no significant inhibitory effect.Conclusion:The serum HDL level in pSS patients was significantly reduced,and the HDL level was significantly negatively correlated with the inflammation level in pSS patients.
