摘 要:目的分析中国水痘流行病学特征。方法通过中国疾病预防控制信息系统收集中国2016-2019年水痘病例和突发公共卫生事件(Public health emergency event,PHEE),进行描述性流行病学分析。结果2016-2019年中国共报告水痘3047715例,年均报告发病率为55.05/10万;发病率从2016年的35.50/10万上升到2019年的70.14/10万;各省份年均发病率在19.66/10万(黑龙江省)-117.17/10万(江苏省)之间;每年5-7月和10月-次年1月为发病高峰;0-4岁、5-9岁、10-14岁、15-19岁、≥20岁年均发病率(/10万)分别为61.77、308.14、228.31、97.69、12.43。共报告水痘PHEE4183起,其中发生在幼儿园、小学、中学分别占12.93%、68.40%、14.61%。结论中国2016-2019年水痘报告发病率呈逐年增高趋势,儿童和学生为高风险人群;建议提高适龄儿童2剂次水痘疫苗接种率,加强水痘监测。Objective To determine epidemiological characteristics of varicella in China. Methods We obtained varicella cases and public health emergency events(PHEEs) reported to the China Disease Prevention and Control Information System during 2016-2019 and conducted a descriptive epidemiological analysis. Results A total of 3 047 715 varicella cases were reported during 2016-2019 in China, yielding an annual incidence of 55.05 cases per 100 000 population. The incidence per 100 000 increased from 35.50 in 2019 to 70.14 in 2019. By province, average annual incidence per 100 000 ranged from 19.66(Heilongjiang) to 117.17(Jiangsu). Yearly incidences peaked between May and July and between October and the following January. Incidences per 100 000 for 0-4, 5-9, 10-14, 15-19, and ≥20-year-olds were 61.77, 308.14, 228.31, 97.69, and 12.43, respectively. A total of 4 183 varicella PHEEs were reported, among which 12.93%, 68.40%, and 14.61% occurred in kindergartens, primary schools, and secondary schools. Conclusions The incidence of varicella showed an increasing trend during 2016-2019 in China, with a high-risk population being children and students. We should increase two-dose varicella vaccination for age-eligible children and enhance varicella surveillance.
