解毒祛瘀抗癌方对原发性肝癌介入术后的影响及其机制探讨

摘　　要：研究分析解毒祛瘀抗癌方治疗原发性肝癌(气滞血瘀证)经导管肝动脉化疗栓塞术(TACE)后的治疗效果及其相关机制探讨。将120例诊断为原发性肝癌(气滞血瘀证)的患者随机分为观察组和对照组,每组各60例,对照组选取单纯西医TACE治疗,观察组除了运用TACE治疗以外还加用解毒祛瘀抗癌方治疗,比较两组患者治疗前与治疗后肝功能、甲胎蛋白(AFP)、凝血功能、转化生长因子-β1(TGF-β1)、肿瘤坏死因子-α(TNF-α)、相关血清指标基质金属蛋白酶(MMPs)、血管内皮生长因子(VEGF)、免疫球蛋白(IgG、IgA)、自然杀伤细胞(NK)、CD4+T淋巴细胞的变化。观察组患者在肝功能、AFP、凝血功能、TGF-β1、TNF-α、MMP-2、MMP-9、VEGF水平的改善情况上与IgG、IgA、NK、CD4+T淋巴细胞的指标含量显著高于对照组,且均优于对照组(P<0.05)。在TACE等西医治疗的基础上,加用解毒祛瘀抗癌方治疗原发性肝癌(气滞血瘀证)的临床疗效较好,能一定程度提高患者生存质量,改善患者的生化指标、个别肿瘤标志物指标、凝血功能、相关细胞因子水平、相关血清指标含量等,通过调节机体免疫功能来抑制肿瘤细胞生长及提高生存率。To study and analyze the therapeutic effect of Jiedu Quyu Kangai Recipe on primary liver cancer(qi stagnation and blood stasis syndrome)after transcatheter hepatic arterial chemoembolization(TACE)and its related mechanism.120 patients with primary liver cancer(qi stagnation and blood stasis syndrome)were randomly divided into observation group and control group,with 60 cases in each group.The control group was treated with TACE alone,while the observation group was treated with Jiedu Quyu anticancer recipe in addition to TACE treatment.Liver function,alpha fetoprotein(AFP),coagulation function and transforming growth factor(TGF)were compared between the two groups before and after treatment-The changes ofβ1(TGF-β1),tumor necrosis factor-α(TNF-α),matrix metalloproteinases(MMPs),vascular endothelial growth factor(VEGF),immunoglobulins(IgG,IGA),natural killer cells(NK)and CD4+T lymphocytes were detected.The improvement of liver function,AFP,coagulation function,TGF-β1,TNF-α,MMP-2,MMP-9,VEGF levels and the index content of IgG,IgA,NK,CD4+T lymphocytes in the observation group were significantly higher than those in the control group,and were better than those in the control group(P<0.05).On the basis of TACE and other Western medicine treatment,the clinical efficacy of Jiedu Quyu Kangai Recipe in the treatment of primary liver cancer(qi stagnation and blood stasis syndrome)is better,which can improve the quality of life of patients to a certain extent,improve the biochemical indicators,individual tumor markers,coagulation function,related cytokines levels,related serum index content,and it can inhibit the growth of tumor cells and improve the survival rate by regulating the immune function of the body.
