健脾合剂对腹泻型肠易激综合征患者肠道菌群的影响

摘　　要：目的:阐述腹泻型肠易激综合征(Irritable Bowel Syndrome,IBS)患者采用聚克(复合乳酸菌胶囊)联合健脾合剂治疗的临床效果及对患者肠道菌群的作用。方法:本次研究时间为2018年4月-2019年10月,研究对象选择我院收治的腹泻型肠易激综合征患者60例,随机将其分为两组,对照组患者单纯予以聚克治疗,观察组患者在此基础上联合健脾合剂治疗,对比两组临床治疗效果,评估比较两组患者治疗前后症状评分、粪便性状评分、患者焦虑评分。治疗前后予以两组肠道基因检测,比较观察肠道菌群结构变化,计算各组患者用药期间发生不良事件的概率。结果:观察组患者此次治疗总疗效明显优于对照组(P<0.05)。两组患者未经治疗前,症状评分、焦虑心态评分等基本相近(P>0.05),经过一段时间的治疗,观察组患者IBS病情严重程度量表(IBS Symptom Severity Scale,IBS-SSS)评分、焦虑评分均低于对照组,且布里斯托粪便性状主分(Bristol粪便性状评分)高于对照组(P<0.05)。观察组治疗后韦荣球菌科、纤维杆菌科、弯曲菌科丰度较对照组明显提高,差异有统计学意义(P<0.05),产碱杆菌科、毛螺旋菌科、理研菌科、瘤胃菌科无明显变化,差异无统计学意义(P>0.05)。两组患者用药后不良事件发生概率无统计学意义(P>0.05)。结论:腹泻型肠易激综合征患者在常规聚克药物基础上联用健脾合剂效果显著,能够更快改善症状表现,调节肠道菌群,且安全性高,提倡推广应用。Objective:To describe clinical effects of lactobacillus complex capsules plus the Jianpi mixture(健脾合剂)on diarrheal irritable bowel syndrome and the intestinal flora.Methods:60 cases in our hospital were randomly divided into two groups.The control group was given lactobacillus complex capsules.The observation group took the Jianpi mixture more.Results:The total effects in the observation group was significantly better than the control group(P<0.05).IBS-SSS scores and anxiety scores in the observation group were lower than those in the control group,and Bristol stool trait scores were higher than those in the control group(P<0.05).After treatment,intestinal floras of patients such as veillonellaceae,cellobacteriaceae and campylobacter in the observation group were significantly more than the control group(P<0.05).Intestinal floras such as alcaligenaceae,lachnospiraceae,rikenellaceae,ruminococcaceae in groups showed no significant change(P>0.05).The incidence of adverse events in the groups showed no statistically significant difference(P>0.05).Conclusion:Lactobacillus complex capsules plus the Jianpi mixture shows a significant effect,can improve the symptoms of patients more quickly,regulate the intestinal flora,with high safety,and is worthy of promotion.
