摘  要:目的监测某肿瘤医院临床分离的肠球菌属细菌的分布和耐药特征。方法回顾性分析2013年—2018年临床分离的肠球菌属细菌的菌种分布、标本来源、科室及耐药情况。结果2013年—2018年共分离出829株非重复肠球菌属细菌,其中粪肠球菌521株(62.85%),屎肠球菌301株(36.31%),主要分布于泌尿生殖道肿瘤科和消化道肿瘤科,以引流液、尿液、伤口脓性分泌物为主要标本来源;屎肠球菌对青霉素类、喹诺酮类、高浓度庆大霉素和呋喃妥因等抗菌药物的耐药性显著高于粪肠球菌(P<0.05),未检出耐万古霉素和利奈唑胺的粪肠球菌和屎肠球菌;不同科室分离的粪肠球菌和屎肠球菌对常用抗菌药物的耐药率不同;尿液中分离的粪肠球菌和屎肠球菌对喹诺酮类耐药率更高(P<0.05)。结论肠球菌属细菌对万古霉素及利奈唑胺仍保持高敏感率,不同科室不同标本的耐药性存在差异,临床医师应合理并个体化使用抗菌药物。Objective To monitor the distribution of Enterococcus in a cancer hospital and its drug resistance characteristics.Methods The pathogens of clinical isolates of Enterococcus from 2013 to 2018 were collected.The distribution,specimen source,department and drug resistance of pathogens were retrospectively analyzed.Results A total of 829 strains of Enterococcus were isolated,including 521 strains of Enterococcus faecalis(62.85%)and 301 strains of Enterococcus faecium(36.31%).The bacteria were mainly isolated from departments of urogenital tumor and gastrointestinal tumor,with drainage fluid,urine and wounds purulent secretion as the main source of specimens.The resistance rates of E.faecium strains to penicillins,quinolones,high concentrations of gentamycin and furantoin were much higher than those of E.faecalis(P<0.05).No E.faecalis and no E.faecium were resistant to vancomycin and linezolid.The resistance rate of E.faecalis and E.faecium isolated from different departments were different to common antibiotics.E.faecalis and E.faecium isolated from urine were more resistant to quinolones(P<0.05).Conclusion Linezolid and vancomycin are still highly active against Enterococcus isolates.Drug resistance characteristics were different between departments and between specimen sources.Clinicians should use antimicrobial agents rationally and individually.
