基于证素辨证的干眼证候评分体系的建立

摘　　要：目的:建立基于证素辨证的干眼证候评分体系。方法:基于证素辨证理论归纳131例干眼患者的证素、证候特点,根据证素、证候特点建立干眼证候评分表。结果:131例干眼患者中常见证候按频率从高到低依次为眼干涩、鼻唇干燥、小便黄、咽干、畏光、情志不舒、口渴、口苦、舌苔干燥、心烦、神疲、目痛、脉数、便秘、灼痛、舌体干燥、失眠、脉细、舌有裂纹、环境干燥、脉沉、发热。Ⅰ级证素共2个,为血虚、血热;Ⅲ级证素共5个,为肝、热、阴虚、燥、津亏。形成的干眼证候评分表共14个评价指标,包括有无眼干涩感、鼻唇干燥感、小便黄、咽干、畏光、情志不舒、口渴、口苦、舌苔或舌体干燥、心烦、视疲劳、目灼痛感、脉数、便秘。结论:相对于干眼症状评分,基于证素辨证的干眼证候评分体系囊括了较多能反应干眼患者症状的中医证候,能更为全面的反应干眼的疾病特征。Objective:To establish a dry eye syndrome scoring system based on syndrome differentiation.Methods:Based on the syndrome differentiation theory,131 patients with dry eye were summarized,and the dry eye syndrome score table was established based on the characteristics of syndromes and symptoms.Results:Syndrome include dry eye,dry nose,yellow urine,dry throat,photophobia,affect-mind dissatisfaction,thirst,bitterness,dry tongue,being perturbed,mind fatigue,eye pain,rapid pulse,constipation,burning pain,dry tongue,insomnia,fine pulse,cracked tongue,dry environment,deep pulse and fever.There were two grade I syndrome elements,which were blood deficiency and blood heat.There were five grade III syndrome elements,which were Liver,heat,Yin deficiency,dryness and loss of body fluids.The dry eye syndrome score table had a total of 14 evaluation indicators,including the presence or absence of dry eyes,dry nose and lip,yellow urine,dry throat,photophobia,affect-mind dissatisfaction,thirst,bitter mouth,dry tongue coating or dry tongue body,being perturbed,photophobia,eyeburn,rapid pulse and constipation.Conclusion:Compared to the dry eye symptom scores,the dry eye syndrome scoring system based on syndrome differentiation contains more TCM syndrome elements that can reflect the symptoms of dry eye,and it can comprehensively reflects the disease characteristics.
