循经取五腧穴针刺疗法对湿热蕴结型痛风性关节炎患者的临床疗效

摘　　要：目的:探究循经取五腧穴针刺疗法对湿热蕴结型痛风性关节炎患者临床疗效和对血清炎症因子的影响。方法:选择符合纳入标准的101例痛风性关节炎患者,采用随机数字表将患者分为对照组和观察组,对照组(50例)口服依托考昔结合红外线灯照射治疗,观察组(51例)在对照组治疗基础上联合循经取五腧穴针刺疗法治疗,比较两组患者临床治疗效果、治疗前后两组患者中医证候积分变化及实验室指标(UA、ESR、CRP)水平变化、血清炎症因子水平变化、患者不良反应情况。结果:观察组治疗总有效率(96.08%)高于对照组(84.00%)(P<0.05);治疗前,两组患者中医证候积分、血尿素(UA)及血沉(ESR)、C反应蛋白(CRP)、白介素-1(IL-1)、白介素-6(IL-6)、白介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)等血清炎症因子水平比较没有统计学意义(P>0.05);治疗后各组患者中医证候积分、UA、ESR、CRP、IL-1、IL-6、IL-8、TNF-α等指标均显著改善,观察组患者治疗后中医证候积分、ESR、CRP、IL-1、IL-6、IL-8、TNF-α等指标均优于对照组(P<0.05);观察组患者未见不良反应,对照组出现1例皮肤过敏(P>0.05)。结论:循经取五腧穴针刺疗法可显著提升湿热蕴结型痛风性关节炎患者临床疗效,降低炎症因子水平,未见不良反应,安全可靠,值得推广与应用。Objective:To explore the effect of needling five-shu acupoints on serum inflammatory factors in the treatment of gout arthritis of dampness-heat accumulation.Methods:101 cases of gouty arthritis met the inclusion criteria were randomly divided into the control group(n=50)and the observation group(n=51).The control group was treated with Etoricoxib in combination with infrared light therapy;on which basis,the observation group was also treated with needling five-shu acupoints.The clinical effect,the changes of TCM syndrome integral,laboratory indexes levels,serum levels of inflammatory factors,as well as adverse reactions were compared between the two groups.Results:The total effective rate was 96.08%(49/51)in the observation group,which was significantly higher than 84.00%(42/50)in the control group(P<0.05).Before the treatment,there were no statistical differences in the TCM syndrome integral(joint pain,joint swelling,limited activity,fever,thirst and prefer drinking,upset and irritated,dark urine,red tongue,yellow greasy coating,slippery pulse),in the levels of blood urea(UA)and blood sedimentation(ESR),c-reactive protein(CRP),interleukin 1(IL-1),interleukin 6(IL-6),interleukin 8(IL-8),tumor necrosis factor alpha(TNF-α)between the two groups(P>0.05);which were significantly improved after the treatment in the two groups,and the improvements in the observation group were more significant(P<0.05).No adverse reactions were found in the observation group,and 1 case of skin allergy appeared in the control group;there were no statistical difference between the two groups(P>0.05).Conclusion:Needling five-shu acupoints can significantly improve the clinical efficacy of patients with gout arthritis of dampness-heat accumulation,it can improve the symptoms without adverse reactions.The therapy is safe and reliable,which is worthy of promotion and application.
