摘 要:目的探讨低纬高原地区昆明市部分学龄前听力正常儿童中耳功能正常参考值范围。方法以2017年3月至2018年3月间行常规体检且听力正常的昆明地区(包括昆明主城区、宜良县、晋宁县、石林县)2 716例(5 432耳)3~6岁儿童为研究对象,均进行双耳声导抗测试,获得外耳道容积、中耳压(绝对值)、声顺值及梯度四项指标的平均值及参考值范围,并比较不同性别、区域、耳别和年龄者的差异。结果昆明地区学龄前儿童外耳道容积平均值为0.74±2.22 ml[95%参考值范围(reference interval,CI)0.67~0.80],声顺值为0.60±5.55 ml(95%CI:0.44~0.76),中耳压为-38.71±79.58 daPa(95%CI:-40.99^-39.43),梯度为148.04±1.09 daPa(95%CI:145.91~150.17)。四个区域男童外耳道容积和声顺值大于女童;宜良县男、女童的左耳外耳道容积大于右耳;昆明主城区男童中耳压随年龄增长而降低,宜良县男童中耳压随年龄增长而增高,石林县男童外耳道容积随年龄增加而变大,压力、声顺值及梯度比较差异有统计学意义(均为P<0.001)。结论本组低纬高原地区学龄前听力正常儿童中耳功能正常值与已报道的非高原地区相应数值比较,外耳道容积均值无明显差异,但中耳压、声顺值均值与浙江地区(分别为-183~26 daPa、0.32,P_5~P_(95):0.14~0.66)相比范围不同。本研究所获得参考值仍处在推荐参考值范围内,但是区间的上下限数值与推荐参考值范围相比明显缩小。Objective To investigate the normal reference value range of middle ear function in preschool children(3~6 years old)with normal hearing in low latitude plateau in Kunming area.Methods Children aged 3~6 years(except hearing abnormalities and ear disorders)from March 2017 to March 2018 were included in this study.Data of ear canal volume,middle ear pressure,compliance,and gradient were recorded.Different distribution was compared by gender,region,age,and two sides of ear.Results The external ear canal volume of preschool children in Kunming was 0.74±2.22 ml[95%reference interval(CI)0.67 to 0.80],and the acoustic compliance was 0.60±5.55 ml(95%CI:0.44 to 0.76).Middle ear pressure was-38.71±79.58 daPa(95%CI:-40.99 to-39.43),and the gradient was 148.04±1.09 daPa(95%CI was 145.91 to 150.17).The external ear canal volume and acoustic compliance of boys was larger than those of girls'.The volume of the left ear canal of boys in Yiliang County was larger than that of the right ear,and the volume of the left ear canal of girls in Yiliang County was larger than that of the right ear.The middle ear pressure of boys in the main urban area decreased with age,while the middle ear pressure of boys in Yiliang County increased with age,and the external ear canal volume of boys in Shilin District also increased with age.There were statistical differences between different groups due to different regions and ages.Conclusion The comparison of the middle ear parameters in Kunming and the reported middle ear parameters of school-age children in non-plateau areas show that the mean volume of ear canal has no significant difference between the two regions,but the comparison of middle ear pressure in Kunming(-40.99 to-39.43 daPa)and Zhejiang(-183 daPa^26 daPa),and the comparison of the gradient in Kunming(0.6±0.55 ml)and Zhejiang(0.32,P 5-P 95:0.14~0.66)seem have different range.The reference range in Kunming obtained by this study is still within the recommended reference range,but the upper and lower limits of the interval are sig
