摘 要:目的探讨1 000 Hz探测音声导抗检查在听力筛查未通过新生儿听力评估中的作用。方法回顾性分析2018年1月至2019年12月期间新生儿听力筛查未通过的531例(820耳)婴儿的临床资料,其中DPOAE及AABR均未通过者337例527耳为A组,DPOAE未通过但AABR通过者192例289耳为B组,DPOAE通过但AABR未通过者2例4耳为C组,以DPOAE及AABR均通过的1 000例(2 000耳)为对照组(D组),对四组新生儿行1 000 Hz探测音声导抗测试,分析各组Ya图形态及分型。结果四组1 000 Hz声导抗检查Ya图分型比较有显著差异,A组和B组异常率(分别为95.07%和85.81%)显著高于对照组(1.16%.),差异有统计学意义(P<0.05),C组2例4耳均为正常型。结论对新生儿听力初筛或复筛未通过的新生儿,再采用1 000 Hz探测音声导抗检查可早期判断新生儿中耳功能,降低DPOAE及AABR听力筛查的假阳性率。Objective To study the clinical characteristics of newborns who failed to pass the primary hearing screening by 1000 Hz probe sound impedance test,and to clarify the application value of 1000 Hz probes to the newborn hearing evaluation.Methods A retrospective analysis was conducted.The clinical data of newborns who completed hearing tests in our hospital from January 2018 to December 2019 were collected.Among them,337 neonates(527 ears)referedboth by DPOAE and AABR in the hearing screening were included in group A,192 neonates(289 ears)refered by DPOAE but passed by AABR were included in group B,2 neonates(4 ears)passed by DPOAE but refered by AABR were included in group C,1000 newborns(2000 ears)passed both by DPOAE and AABR were selected and included in group D as a control group.Recorded and compared Ya curve of tympanogramwith 1000 Hz acoustic immittance test were carried out among the four groups.Results There was a significant difference in the classification of Ya curve pattern in the four groups with 1000 Hz acoustic immittance test.The proportion of abnormal patterns in group A(95.07%)and B(85.81%)was higher than group D(1.16%),and the difference was statistically significant(P<0.05).Conclusion In clinical newbornprimary and secondary hearing screening,1000 Hz acoustic immittancetest can be selected according to the individual condition of the newborn.It can be used to evaluate the middle ear function of newborns,which can be greatly reduced false-positive rate by DPOAE and AABR methods.
