摘 要:目的研究T2DM患者胰岛素原(PI)、真Ins(TI)与IR的相关性,分析其对IR的影响。方法选取2018年12月至2019年2月于徐州医科大学附属医院内分泌科住院治疗的107例T2DM住院患者,根据BMI分为体重正常组(NW,n=36)、超重组(OW,n=39)及肥胖组(OB,n=32)。检测FPG、血脂各项、免疫反应性Ins(IRI)、PI、TI等。结果与NW、OW组比较,OB组PI、TI、胰岛素抵抗指数(HOMA-IR)升高(P<0.05);各组HOMA-IR与PI(r=0.576、0.486、0.538)、TI(r=0.905、0.867、0.908)呈正相关(P<0.05)。多元逐步回归分析显示,TI、PI、IRI、FPG、HDL-C是HOMA-IR的影响因素(P<0.05);偏相关分析结果显示,HOMA-IR与TI、PI呈正相关(r=0.930、0.612,P<0.01)。结论T2DM患者PI、TI均参与IR,可能是IR的影响因素。Objective To investigate the relationship between proinsulin(PI),true insulin(TI)and insulin resistance(IR)in patients with type 2 diabetes mellitus(T2DM).Methods A total of 107 T2DM hospitalized patients were selected.They were divided into three groups randomly,including normal weight group(NW,n=36),overweight group(OW,n=39)and obesity group(OB,n=32).Fasting plasma glucose(FPG),blood lipid,immunoreactive insulin(IRI),PI,TI and other indicators were mea-sured.Results Compared with NW group and OW group of T2DM,PI,TI and insulin resistance index(HOMA-IR)were increased in OB group.Both P(Ir=0.576,0.486,0.538,P<0.001)and TI(r=0.905,0.867,0.908,P<0.001)were significantly correlated with HOMA-IR in T2DM patients.Multiple step-wise regression analysis showed that TI,PI,IRI,FPG and high density lipoprotein cholestero(lHDL-C)were the influencing factors of HOMA-IR.Partial correlation analysis found that PI(r=0.612,P<0.01)and TI(r=0.930,P<0.01)were positively correlated with HOMA-IR.Conclusion TI and PI may both participate and affect the development of insulin resistance in T2DM patients.
