胰岛素与胰岛素类似物用于胰岛素泵联合二甲双胍对新诊断2型糖尿病患者治疗的临床疗效及药物经济学对比观察

摘　　要：目的比较Ins及类似物用于胰岛素泵给药(CSⅡ)联合二甲双胍治疗新诊断T2DM的临床疗效与经济性。方法选取2018年1月至2019年2月于我院内分泌科住院治疗的100例新诊断T2DM患者,随机分为诺和灵组(NL组)和诺和锐组(NR组),每组各50例。两组均采用CSⅡ联合二甲双胍治疗,比较临床疗效、低BG发生率及成本效果比(C/E)。结果NR、NL组FBG控制时间分别为(4.13±0.34)和(5.31±0.67)d,2 hBG控制时间分别为(5.06±0.53)和(6.07±0.84)d,差异均有统计学意义(P<0.05)。FBG、2 hBG下降1%的成本中NR组C/E分别为21.76元、23.42元,低于NL组32.73元、35.75元。结论使用Ins及类似物用于CSⅡ联合二甲双胍缓释片治疗新诊断T2DM患者可获得较好的临床疗效,胰岛素类似物疗效更佳,经济学效益更高。Objective To compare the clinical efficacy and pharmacoeconomics between combination therapy of insulin and insulin analogues with insulin pump(CSⅡ)and Metformin in patients with newly diag-nosed type 2 diabetes mellitus(T2DM).Methods A total of 100 hospitalized patients with T2DM were enrolled in this study from the Endocrinology department of our hospital from January 2018 to February2019,and randomly divided into two groups:Novolin group(NL group,n=50)and NovoRapid group(NR group,n=50).All patients were treated with CSⅡcombined with Metformin.The clinical efficacy,hypoglycemia rate and cost-effectiveness ratio(C/E)were compared between the two groups.Results The fasting blood glucose(FBG)control time was(4.13±0.34)and(5.31±0.67)d,and the 2 h post-prandial blood glucose(2 hBG)control time was(5.06±0.53)and(6.07±0.84)d in NR group and NL group respectively.The difference was statistically significant(P<0.05).The C/E value of FBG and 2 hBG decreased by 1%were 21.76 yuan and 23.42 yuan in NR group,which was significantly lower than NL group(32.73 yuan and 35.75 yuan).Conclusion The use of insulin and insulin analogues in CSⅡcom-bined with Metformin in T2DM can both achieve favorable clinical efficacy.However,insulin analogues show better efficacy and economic benefits.
