五步检查法在胎儿心脏畸形产前超声筛查中的应用价值

摘　　要：目的探究五步检查法在胎儿心脏畸形产前超声筛查中的应用价值。方法选取2017年2月至2019年10月于本院产检且疑似胎儿心脏畸形的102例孕妇作为研究对象,随机分为实验组和对照组,每组51例。对照组给予常规产前超声检查法,实验组给予五步检查法进行胎儿心脏畸形筛查。比较两组胎儿心脏超声检查参数差异、检查准确率以及诊断方法敏感率。结果实验组四腔心切面、左右心室流出道切面、三血管切面检查有效率均高于对照组(P<0.05);实验组漏诊2例,诊断准确率为96.08%,诊断准确率高于对照组(P<0.05);实验组敏感率为90.18%,产后诊断敏感率为78.40%,两组孕妇比较差异有统计学意义(P<0.05)。结论五步检查法在胎儿心脏畸形产前超声筛查中的应用价值较高,能有效提升胎儿心脏畸形诊断率,有益于胎儿存活率和健康出生率,值得临床推广应用。Objective To explore the application value of five step examination in prenatal ultrasound screening of fetal heart malformation.Methods 102 parturients with suspected fetal heart malformation who were examined in our hospital from February 2017 to October 2019 were randomly divided into experimental group and control group,with 51 cases in each group.The control group was given routine prenatal ultrasound,and the experimental group was given five-step examination for fetal heart malformation screening.The difference of ultrasonic parameters,accuracy and sensitivity of diagnostic methods were compared between the two groups.Results In the experimental group,five-step ultrasonography was applied in the four-chamber heart section,left and right ventricular outflow tract section,and three-vessel section,all of which were higher than the control group(P<0.05).There were 2 cases of missed diagnosis in the experimental group,and the diagnostic accuracy was 96.08%,which was higher than that in the control group(P<0.05).The sensitivity rate of the five-step test was 90.18%,and the sensitivity rate of postpartum diagnosis was 78.40%,the difference was statistically significant between the two groups(P<0.05).Conclusion Five step examination is of great value in prenatal ultrasound screening of fetal heart malformation.It can effectively improve the diagnosis rate of fetal heart malformation,and is beneficial to fetal survival rate and healthy birth rate,it is worth popularizing in clinical practice.
