血清β-HCG和孕酮联合检测在早期先兆流产的临床诊断意义

摘　　要：目的探讨血清β-HCG和孕酮联合检测在早期先兆流产的临床诊断意义。方法选择2017年12月~2019年12月我院的120例先兆流产孕妇作为观察组,根据临床结局分为继续妊娠组(68例)和妊娠失败组(52例),选取同期的120例正常妊娠孕妇作为对照组。检测所有孕妇的血清β-绒毛膜促性限激素(β-HCG)和孕酮水平,分析血清β-HCG和孕酮对早期先兆流产的诊断效能。结果继续妊娠组和妊娠失败组孕妇的血清β-HCG和孕酮水平低于对照组(均P<0.01);继续妊娠组的β-HCG和孕酮水平高于妊娠失败组(均P<0.01)。Spearman相关性分析显示,血清β-HCG和孕酮水平均与早期先兆流产存在负相关(rsβ-HCG=-0.846,rs孕酮=-0.694);β-HCG对应的ROC曲线下面积为0.896,孕酮对应的ROC曲线下面积为0.748,β-HCG+孕酮联合诊断对应的ROC曲线下面积为0.948。结论血清β-HCG和孕酮水平在先兆流产孕妇中变化明显,血清β-HCG和孕酮联合检测对早期先兆流产诊断具有重要价值。Objective To explore the clinical diagnosis significance of combined detection of serumβ-human chorionic gonadotrophin(β-HCG)and progesterone in early threatened abortion.Methods A total of 120 pregnant women with threatened abortion in our hospital from December 2017 to December 2019 were selected as the observation group,and they were further divided into the continuing pregnancy group(68 cases)and the pregnancy failure group(52 cases)according to clinical outcome.A total of 120 normal pregnant women in the same period were selected as the control group.Serumβ-HCG and progesterone levels in all pregnant women were detected and the diagnostic efficacy of serumβ-HCG and progesterone in early threatened abortion were analyzed.Results The serumβ-HCG and progesterone levels of the pregnant women in the continuing pregnancy group and the pregnancy failure group were lower than those in the control group(both P<0.01).The levels ofβ-HCG and progesterone in the continued pregnancy group were higher than those in the pregnancy failure group(both P<0.01).Spearman correlation analysis showed that the serumβ-HCG and progesterone levels were negatively correlated with early threatened abortion(rsβ-HCG=-0.846,rs progesterone=-0.694).The area under the ROC curve corresponding toβ-HCG was 0.896,the area under the ROC curve corresponding to progesterone was 0.748,and the area under the ROC curve corresponding toβ-HCG+progesterone diagnosis was 0.948.Conclusion Serumβ-HCG and progesterone levels change significantly in pregnant women with threatened abortion.The combined detection of serumβ-HCG and progesterone is of great value in the diagnosis of early threatened abortion.
