序贯经鼻高流量通气在急诊重症呼吸衰竭患者护理工作中的应用效果观察

摘　　要：目的探讨序贯经鼻高流量通气在急诊重症呼吸衰竭患者护理拔管后的临床应用效果。方法选取急诊重症呼吸衰竭患者82例,根据随机数字表法分为对照组和研究组,各41例。对照组予以有创机械通气治疗的方式,研究组予以序贯经鼻高流量通气治疗的方式。比较2组者在拔管后20 min、50 min、100 min血气分析相关指标情况、治疗过程中监护仪数据的变化情况及进行治疗后的转归情况。结果研究组在拔管后20 min、50 min、100 min的SpO2值、PaO2值均显著高于对照组,呼吸频率、PaCO2值均显著低于对照组(P<0.05);研究组再插管率、死亡率、住院时间均显著低于对照组(P<0.05)。结论序贯经鼻高流量通气对急诊重症呼吸衰竭患者可以发挥良好的改善作用,在临床中可以推广于呼吸衰竭患者护理中。Objective To explore the clinical effect of sequential transnasal high flow ventilation after extubation in emergency care for patients with severe respiratory failure.Methods A total of 82 patients with emergency severe respiratory failure were divided into control group and study group,41 cases each.The control group was treated with invasive mechanical ventilation,and the study group was treated with sequential transnasal high flow ventilation.Blood gas analysis related indexes of 20 min,50 min,100 min after extubation,the change of monitor data during treatment and the outcome after treatment were compared.Results The SpO2 and PaO2 values of 20 min,50 min,100 min after extubation in the study group were significantly higher than those in the control group,and the respiratory frequency and PaCO2 values were significantly lower than those in the control group(P<0.05);re-intubation rate,mortality rate and hospitalization time in the study group were significantly lower than those in the control group(P<0.05).Conclusion Sequential high flow ventilation through the nose can play a good role in the emergency patients with severe respiratory failure.
