摘  要:目的探讨品管圈精益管理方式在提高肿瘤标志物检验项目西格玛水平中的应用价值。方法2019年6月至12月在新疆医科大学附属肿瘤医院检验科开展针对如何提高肿瘤标志物检验项目西格玛水平的品管圈活动,按照品管圈管理模式,成立由9名检验科年轻职工参与的品管圈,按照品管圈步骤,通过选定主题、拟定对策、对策实施、结果评价等步骤逐步开展。结论品管圈活动中通过细化肿瘤标志物检验项目室内质量控制的操作流程,强化室内质量控制操作人员的培训与考核,重视所有项目室间质评的结果分析,优化各项目的校准频率,加强仪器的维护保养,最终使科室10项西格玛水平小于6的肿瘤标志物检验项目的西格玛水平均提高了一个档次,达到了设定目标,并完成了科室定量检验项目西格玛水平管理办法的制定。同时各圈员的能力都有明显提高。品管圈手法提高86.4%、团队精神提高50.0%、专业知识提高23.5%、沟通协调提高40.0%、活动信心提高60.0%、责任荣誉提高17.1%。结论采用品管圈管理方式可以有效提高肿瘤标志物检验项目西格玛水平,同时提高全员对品管圈管理方式的认知水平,品管圈管理模式作为自主精益化管理手段值得在检验科推广。Objective To explore the application value of the management method of QCC-Quality Control Circle in improving the Sigma of tumor marker test.Methods We carried out the QCC-Quality Control Circle on improving the Sigma of tumor marker test in the Tumor Hospital affiliated to Xinjiang Medical University from June to December in 2019.According to the quality control management mode,we set up a team with 9 young workers to participate in the clinical laboratory quality management circle.According to the QCC steps,as well as through the selected topic,proposed countermeasures,and countermeasures for implementation,the result evaluation steps were carried out step by step.Results In the activities of QCC,through refining the operation flow of the IQC of the tumor markers,we strengthened the train and assessment of the IQC operators,and paid attentions to the analysis for the results of EQA,optimized the calibration frequency of every item,and strengthened the maintenance of the instrument.With these steps,the 10 tumor marker testing projects with sigma level less than 6 in the department were improved by one level,and the set goals were achieved.The sigma value management method for quantitative testing projects in the department was established.At the same time,the ability of each circle member was significantly improved.Quality management practices were improved by 86.4%,team spirit by 50.0%,professional knowledge by 23.5%,communication and coordination by 40.0%,activity confidence by 60.0%,and responsibility and honor by 17.1%.Conclusion The QCC management method can effectively improve the sigma level of tumor marker test and enhance the cognition level of the whole staff on the QCC management mode.The QCC management mode is worthy to be popularized in the clinical laboratory department.
