血小板聚集率与冠心病患者病情严重程度的关系研究

摘　　要：背景临床常采用Gensini积分评估冠心病(CHD)患者病情严重程度,但该评分项目因成本较高而导致临床应用受限。血小板聚集功能检测简便、易于开展,推测其与CHD患者病情严重程度有关。目的探讨血小板聚集率与CHD患者病情严重程度的关系,以为临床治疗提供参考。方法选取2016年1月—2019年1月青海省红十字医院心血管内科诊治的CHD患者162例。收集患者的临床资料、血小板聚集率、Gensini积分。采用Pearson相关分析或Spearman秩相关分析CHD患者血小板聚集率与临床资料、Gensini积分的相关性;采用多元线性逐步回归分析CHD患者病情严重程度的影响因素。结果Pearson相关分析或Spearman秩相关分析结果显示,CHD患者血小板聚集率与年龄(r=0.568)、吸烟(rs=0.335)、糖化血红蛋白(HbA1c)(r=0.498)、Gensini积分(r=0.781)呈正相关,与高密度脂蛋白胆固醇(HDL-C)(r=-0.325)呈负相关(P<0.05)。多元线性回归分析结果显示,年龄(β=0.041)、LVEF(β=0.265)、血小板聚集率(β=0.219)是CHD患者病情严重程度的影响因素(P<0.05)。结论血小板聚集率可以作为评价CHD患者病情严重程度的候选指标。Background Gensini score is a commonly used clinical index for the assessment of the severity of the coronary heart disease(CHD),but it is not convenient to use due to the complex evaluation procedure involved.This study hypotheses that the platelet aggregation rate is correlated with the severity of the CHD and the evaluation procedure is much simpler and easier.Objective To study the correlation between the platelet aggregation rate and the severity of the CHD,as a guidance for the clinical assessment of the CHD.Methods A total of 162 cases of CHD patients were enrolled for the study in the Qinghai Red Cross Hospital from January 2016 to January 2019.Baseline characteristics,the platelet aggregation rate,and the Gensini score were collected.Pearson correlation analysis or Spearman rank correlation analysis was conducted to evaluate the correlation between the platelet aggregation rate and baseline characteristics,and the Gensini score.Influencing factors of the severity of CHD were analyzed by multiple linear stepwise regression analysis.Results Pearson correlation analysis or Spearman rank correlation analysis showed that the platelet aggregation rate in CHD patients was positively correlated with the age(r=0.568),smoking(rs=0.335),HbA1c(r=0.498),and the Gensini score(r=0.781),but negatively correlated with the HDL-C(r=-0.325)(P<0.05).Multiple linear regression results showed that the age(β=0.041),LVEF(β=0.265),and platelet aggregation rate(β=0.219)were the prominent influencing factors of the severity of the CHD(P<0.05).Conclusion Platelet aggregation rate can be used as a clinical candidate index for the assessment of the severity of the CHD.
